Since 1977, TSLI has operated the Pathways Program which has provided safe, stable housing and support services to its approximately 125 residents. The supervised living programs, located in Smithtown, Holtsville, Central Islip and on the grounds of Pilgrim Psychiatric Center in Brentwood, provide 24-hour supervision and support to residents. The apartment program sites, located in Brentwood, Holtsville, Port Jefferson and Ronkonkoma are staffed 7 days a week and key staff are available via telephone after business hours. All applicants are referred to the program via a single point of access. The residents are assisted in a variety of skill areas in addition to housing support and supervision. Upon entry, residents work with staff to create a service plan which focuses on their discharge goal of more independent living. Goals and objectives are then established to help residents develop the skills needed to reach their discharge criteria. They work both individually, as well as in groups, to gain and maintain the skills necessary to become active community participants. Residents receive assistance in such areas as assertiveness training, community integration, health services, medication management, money management and socialization to name a few. The organization also provides job training and personal skill development to mentally ill persons pursuing reintegration into the competitive workforce in order to obtain an increased level of self-sufficiency.
