SANTA BARBARA NEIGHBORHOOD CLINICS (SBNC) WAS FORMED IN 1998 THROUGH A MERGER OF THREE EXISTING MEDICAL CLINICS, WHICH HAD PROVIDED MEDICAL CARE TO THE UNDERSERVED POPULATIONS SINCE THE 1970'S. IN 2013, SBNC WAS AWARDED A NEW ACCESS POINT DESIGNATION FROM THE HEALTH RESOURCES AND SERVICES ADMINISTRATION, DESIGNATING THE GOLETA EXPANSION SITE AND SERVICES AS A SECTION 330(E) FEDERALLY QUALIFIED HEALTH CENTER (FQHC) UNDER SECTION 330(E) OF THE PUBLIC HEALTH SERVICE ACT. THE EASTSIDE NEIGHBORHOOD MEDICAL/ EASTSIDE NEIGHBORHOOD FAMILY DENTAL, WESTSIDE NEIGHBORHOOD, AND ISLA VISTA NEIGHBORHOOD CLINICS WERE PREVIOUSLY OPERATED UNDER LOOK ALIKE STATUS WITHIN THE RULES OF AN FQHC, IN CONJUNCTION WITH CODE SECTION 330(E) STANDARDS WERE BROUGHT INTO THE FULL 330(E) SCOPE OF PRACTICE FOR AN FQHC. SBNC IS COMPRISED OF FOUR PRIMARY CARE CLINICS, TWO DENTAL CLINICS, A "BRIDGE" CLINIC DEDICATED TO TREATMENT OF SUBSTANCE USE DISORDERS AND ONE INTREGRATED CARE CLINIC, WHERE INTEGRATED PRIMARY MEDICAL, DENTAL AND BEHAVIORAL HEALTHCARE IS DELIVERED. IN 2004, SBNC ADDED DENTAL HEALTH SERVICES AFTER A LONG PARTNERSHIP WITH THE SANTA BARBARA CHILDREN/FAMILY DENTAL COLLABORATIVE. ADDITIONALLY, HEALTH PROMOTION SERVICES AND PATIENT EDUCATION, ENABLING SERVICES ARE PROVIDED WITHIN THE CLINICS. THESE CLINICS ARE LOCATED IN THE CITIES OF SANTA BARBARA AND GOLETA AND IN ISLA VISTA, AN UNINCORPORATED DISTRICT AND CENSUS-DESIGNATED PLACE, IN THE COUNTY OF SANTA BARBARA, CALIFORNIA. SBNC PROVIDES A WIDE-SPECTRUM OF HEALTH CARE SERVICES AND HAS BEEN A SUB-RECIPIENT OF SANTA BARBARA COUNTY DEPARTMENT OF PUBLIC HEALTH SECTION 330(H) HOMELESS GRANT SINCE 1998.SBNC'S TARGET POPULATION INCLUDE THE LOW-INCOME, UNINSURED, HOMELESS AND UNDERSERVED POPULATIONS (E.G. 63 % OF SBNC PATIENTS ARE LATINO AND 54% HAVE INCOMES BELOW THE FEDERAL POVERTY GUIDELINE) OF SOUTH SANTA BARBARA COUNTY. IN FISCAL YEAR ENDING JUNE 30, 2019, SBNC PROVIDED 56,099, MEDICAL ENCOUNTERS, 28,960, DENTAL ENCOUNTERS, AND 5,921, BEHAVIORAL HEALTH ENCOUNTERS FOR A TOTAL OF, 90,980, CLINICAL VISITS SERVING A TOTAL OF 21,938 UNDUPLICATED PATIENTS. THE MAJOR HEALTH CARE NEEDS OF THE TARGET POPULATION INCLUDE ACCESS TO AFFORDABLE MEDICAL, DENTAL AND BEHAVIORAL HEALTH CARE, BI-LINGUALLY AND CULTURALLY COMPETENT SERVICES. IN ADDITION, A MAJOR HEALTH CARE CONCERN OF OUR PATIENTS IS THE PREVALENCE OF OBESITY AND THE RESULTING DIAGNOSIS, TREATMENT AND MONITORING OF DIABETES- ESPECIALLY IN ADOLESCENTS. AMONG THE TARGET POPULATION IN OUR AREA, 30% ARE SELF-PAY/SLIDING FEE SCALE PATIENTS INCLUDING THE UNINSURED. THE AFFORDABLE CARE ACT HAS DEMONSTRATED AN IMPACT OF INCREASING THE NUMBER OF PATIENTS ELIGIBLE FOR MEDI-CAL SBNC IS SERVING.
