PATIENT EDUCATION AND WELLNESS - CARE COORDINATION, DIABETES EDUCATION, FLU SHOT CLINICS, HEALTHWAYS, PATIENT HEALTH ADVOCACY, AND WELLNESS SCREENINGS. MENTAL HEALTH CARE - COUNSELING, PSYCHIATRIC CARE NAVIGATION, AND PSYCHIATRIC CLINIC. PHARMACEUTICAL SUPPORT - COMMUNITY PHARMACY, MEDI-FIND PRESCRIPTION ASSISTANCE PROGRAM, AND MEDICATION THERAPY MANAGEMENT. CHRONIC AND SPECIALTY CARE - BRIDGES TO HEALTH, COMMUNITY CASE MANAGEMENT/PROJECT ACCESS, DIABETES PROGRAMS, ENDOCRINE CLINIC, AND EYE CLINIC. THE FREE CLINICS REPORTED THE FOLLOWING ACCOMPLISHMENTS DURING THE 2018- 2019 FISCAL YEAR: THE ORGANIZATION PROVIDED 40,399 PATIENT ENCOUNTERS DURING THE YEAR AND FILLED 17,067 PRESCRIPTIONS THROUGH THE COMMUNITY PHARMACY, INCLUDING 1,353 THROUGH PRESCRIPTION ASSISTANCE. THESE SERVICES WERE PROVIDED BY 215 ACTIVE VOLUNTEERS WHO CONTRIBUTED MORE THAN 6,306 VOLUNTEER HOURS. THE ORGANIZATION WAS ABLE TO MAKE 337 REFERRALS TO SPECIALISTS WHO GENEROUSLY PROVIDED FREE CARE AND TO ARRANGE 57 SURGICAL PROCEDURES ON BEHALF OF PATIENTS.
