The Diabetes Care Program, Medication Assistance Program and Kate B. Reynolds Grant assisted with the majority of the Organization's achievements. The Diabetes Care Program is funded through the NC DHHS Division of Public Health. The Medication Assistance Program is funded through NCDHHS Office of Rural Health and provides funding for a prescription assistance advocate. The Kate B. Reynolds Charitable Trust Fund Grant assists with care and coordination of services for uninsured patients and also assists the clinic with funds to support provision of primary care and prescription assistance for resident adults living in Scotland County at or below the poverty level.
