HOSPICE OF THE PRAIRIE DIVISIONThe main purpose of this division is to provide medical care and supportive services to terminally ill patients and their families. These services may be provided in nursing homes, assisted living facilities or in the patient's own home. Hospice services are provided through an interdisciplinary team approach, directed by the patient's own physician. Hospice services allow persons to face the reality of a life-threatening illness with hope and dignity, surrounded by family and friends. Physical, psychological, social and spiritual care are available from the medically directed team consisting of: physician/nursing care, home health aide, social work, pastoral care, volunteers and bereavement.
