Scenic Bluffs, recognized as a patient-centered medical home (PCMH) by the National Committee for Quality Assurance, provides primary, preventive healthcare to the underserved populations of Monroe, Vernon, Crawford, and La Crosse counties in western Wisconsin. In 2018-2019, we served 7,027 patients with a total of 21,712 appointments. Of these appointments, 30% were medical, 53% were dental, 5% were chiropractic, 10% were behavioral health, and many patients received health education and pharmacy services. Scenic Bluffs aims to address social determinants of health through educational, prevention-based programming that connects patients with resources and education. This is possible through our health educators, help team, certified application counselor, and our engagement in nearly 50 community coalitions and boards.
