COVENANT COMMUNITY CARE (COVENANT) USES A PATIENT-CENTERED MEDICAL HOME SERVICE DELIVERY MODEL. WE OFFER COMPREHENSIVE PRIMARY MEDICAL, DENTAL, BEHAVIORAL HEALTH CARE FOR PEOPLE OF ALL AGES, REGARDLESS OF THEIR ABILITY TO PAY. WE HAVE ALSO RECENTLY ADDED A CENTERING PREGNANCY PROGRAM AND AN EYE CLINIC. OUR SIX CLINICAL SITES ARE SPREAD ACROSS METRO DETROIT TO MAKE OUR SERVICES ACCESSIBLE TO A POPULATION WITH SIGNIFICANT FINANCIAL, HEALTH, AND TRANSPORTATION CHALLENGES. CHRONIC CONDITIONS SUCH AS DIABETES, HYPERTENSION, DEPRESSION, ASTHMA, HEART DISEASE, HIV, AND MANY OTHERS ARE MANAGED BY A MULTI-DISCIPLINARY TEAM OF PROVIDERS, NURSES, SOCIAL WORKERS, BENEFIT COORDINATORS, AND VOLUNTEERS. ALL SITES ALSO HAVE A BEHAVIORAL HEALTH COORDINATOR THAT FUNCTIONS TO INTEGRATE THE MENTAL HEALTH SERVICES AND LIFESTYLE EDUCATION NEEDED BY EACH PATIENT TO IDENTIFY AND HELP REMEDY OBSTACLES TO COMPLIANCE, WORKING IN CLOSE ASSOCIATION WITH COMMUNITY MENTAL HEALTH AGENCIES. COVENANT'S SIXTH SITE IS A SCHOOL-BASED CLINIC THAT PROVIDES CRITICAL HEALTH SERVICES YEAR-ROUND TO THE CHILDREN AND FAMILIES OF A NORTHWEST DETROIT SCHOOL AND THE SURROUNDING COMMUNITY. IN 2018, COVENANT SERVED 21,746 PATIENTS THROUGH 73,391 VISITS ACROSS OUR SIX SITES. OF THESE PATIENTS, 26% WERE WITHOUT HEALTH INSURANCE, CREATING GREAT OBSTACLES TO SUCCESSFUL HEALTHY LIVING AND THE MANAGEMENT OF THEIR CHRONIC DISEASES. OUR COMMITMENT TO THOSE MOST IN NEED ENSURES THAT OUR PERCENTAGE OF UNINSURED PATIENTS REMAINS SUBSTANTIAL AS WE CONTINUE TO GROW. THIS CAN PRESENT A CHALLENGE AS THESE SOMETIMES TRANSIENT AND UNDER-RESOURCED CHARACTERISTICS OF THIS GROUP MAKE MEDICAL COMPLIANCE AND FOLLOW UP, ATTAINMENT OF HEALTH GOALS, AND ACCESS TO SERVICES A CHALLENGING GOAL. FOR THIS REASON, WE ARE ALWAYS LOOKING FOR NEW PARTNERS, LIKE COMMUNITY MENTAL HEALTH AGENCIES AND OTHER SOCIAL SERVICE ORGANIZATIONS AND DEVELOPING PROGRAMS, SUCH AS OUR HOMELESS OUTREACH PROGRAM, TO HELP US IMPROVE HOW WE FULFILL OUR MISSION OF "SHOWING AND SHARING THE LOVE OF GOD, AS SEEN IN THE GOOD NEWS OF JESUS CHRIST, BY PROVIDING INTEGRATED, AFFORDABLE, AND QUALITY HEALTH CARE TO THOSE WHO NEED IT MOST."ALSO IN 2018, COVENANT'S TWO NEWEST PROGRAMS WERE ABLE TO SEE CONTINUED GROWTH. THE EYE CLINIC HAD 239 VISITS AND WERE ABLE TO ENJOY ITS NEW CLINIC SPACE AT THE MICHIGAN AVE HEALTH CENTER. THE CENTERING PREGNANCY PROGRAM, WHICH REPLACES TRADITIONAL ONE-ON-ONE VISITS WITH GROUP VISITS AND BRINGS 8-12 WOMEN WHO ARE DUE AT THE SAME TIME TOGETHER FOR PRENATAL CARE, HELPED 100 MOTHERS DELIVER HEALTHY BABIES AND PROVIDE THEM WITH THE TOOLS THEY NEED TO CONTINUE TO BE GREAT PARENTS. COVENANT CONTINUED TO HAVE GROWTH IN QUALITY MEASURES IN 2018, INCLUDING OUR COLORECTAL CANCER SCREENING RATE (INCREASED FROM 47% TO 53%), TWO-YEAR-OLD IMMUNIZATION COMPLIANCE RATE (INCREASED FROM 41.4% TO 47%), PREVENTIVE SCREENING AND CESSASTION INTERVENTION OF TOBACCO USE (INCREASED FROM 67% TO 89%, AND BODY MASS INDEX (BMI) SCREENING AND FOLLOW-UP (INCREASED FROM 62.8% TO 76%).
