THE AGENCY PROVIDED SERVICES TO CLOSE TO 1,000 UNDUPLICATED INDIVIDUALS THROUGHOUT FISCAL YEAR 2019. WE PROVIDE SERVICES IN FOUR MAIN CATEGORIES, ALL WITH THE GOAL OF ASSISTING PEOPLE TO BECOME AS HEALTHY AND INDEPENDENT AS THEY CAN BE. WE OFFER SERVICES TO THOSE ON MEDICAID, WITH INSURANCE AS WELL AS THOSE WHO NEED A SLIDING SCALE DUE TO BEING ECONOMICALLY CHALLENGED. REGARDLESS OF THEIR ABILITY TO PAY, WE DO OUR BEST TO PROVIDE HIGH QUALITY TREATMENT GEARED TO THE SPECIFIC NEEDS OF EACH INDIVIDUAL WE SERVE. SUBSTANCE ABUSE: OUR APPROACH TO ADDICTION TREATMENT INCORPORATES INDIVIDUAL THERAPY AND PSYCHO-EDUCATIONAL GROUP ACTIVITIES AS WELL AS SUPPORT FOR FAMILIES AND FRIENDS OF AN INDIVIDUAL WITH A DRUG OR ALCOHOL PROBLEM. HEARTLAND ALSO PROVIDES EDUCATION AND PREVENTION SERVICES FOR EMPLOYERS AND SCHOOLS IN THE COMMUNITY. HEARTLAND IS CERTIFIED BY THE ILLINOIS DEPARTMENT OF HUMAN SERVICES DIVISION OF ALCOHOLISM AND SUBSTANCE ABUSE (DASA) TO PROVIDE DUI EVALUATIONS AND DUI RISK EDUCATION. ANY TREATMENT RECOMMENDED BY THE EVALUATION CAN BE OBTAINED AT HEARTLAND HUMAN SERVICES. HEARTLAND HUMAN SERVICES PROVIDES TREATMENT FOR ALL AGES, INCLUDING; INDIVIDUALS, FAMILIES, AND COUPLES. WE PROVIDE SERVICES FROM MOST REFERRAL SOURCES, INCLUDING PROBATION/PAROLE, DCFS, EMPLOYER-REFERRED TREATMENT, AND SELF-REFERRAL. MENTAL HEALTH: OUR MENTAL HEALTH TREATMENT IS DESIGNED TO MEET THE NEEDS OF EACH ADULT, YOUTH, OR CHILD. WE OFFER OUTPATIENT COUNSELING FOR PEOPLE OF ALL AGES. THE LENGTH OF COUNSELING VARIES, AND THE NUMBER AND FREQUENCY OF SESSIONS ARE DETERMINED BY EACH CONSUMER'S UNIQUE SITUATION. HEARTLAND IS AVAILABLE TO PROVIDE EDUCATION AND PREVENTION SERVICES IN THE SCHOOLS AND COMMUNITY. WE ACCEPT REFERRALS FROM MANY SOURCES INCLUDING, SELF-REFERRAL, DCFS AND OTHER SOCIAL SERVICE PROVIDERS, PROBATION/PAROLE, AND EMPLOYER-REFERRED TREATMENT. COMMUNITY INTEGRATED LIVING SERVICES: HHS OFFERS 24-HOUR RESIDENTIAL LIVING SERVICES TO ADULTS SUFFERING FROM PERSISTENT MENTAL ILLNESS. THE SUPPORTIVE LIVING SERVICES THAT WE PROVIDE TEACH THEM THE SKILLS NECESSARY FOR THEM TO MOVE TOWARD THE GOAL OF LIVING INDEPENDENTLY IN THE COMMUNITY. WE ALSO PROVIDE OUTPATIENT PROGRAMS FOR THOSE WHO LIVE ON THEIR OWN BUT NEED CASE MANAGEMENT, BUDGETING AND NURSING SERVICES. OLDER ADULT PROGRAMS: INFORMATION AND ASSISTANCE IS PROVIDED TO THOSE AGES 60+. LINKAGE AND REFERRAL IS GIVEN TO OTHER SUPPORTIVE SERVICES THAT HELP KEEP SENIORS LIVING IN THEIR OWN HOMES. THE SENIOR HEALTH INSURANCE PROGRAM (SHIP) PROVIDES INSURANCE COUNSELING FOR MEDICAID AND MEDICARE, INCLUDING PART D. THE FAMILY CAREGIVER PROGRAM HELPS THE CAREGIVER AND THE CARE RECEIVER BY PROVIDING EDUCATION, SUPPORT, COUNSELING, AND RESPITE. SERVICES ARE AVAILABLE IN CLAY, EFFINGHAM, FAYETTE, JEFFERSON, AND MARION COUNTIES. HHS HAS A CONTINUOUS QUALITY IMPROVEMENT TEAM THAT MEETS MONTHLY TO REVIEW POLICIES, JOB DESCRIPTIONS, ADVERSE INCIDENTS AND KEY METRICS. HHS IS LICENSED, REGULATED AND AUDITED BY THE STATE OF ILLINOIS AND ACCREDITED BY THE JOINT COMMISSION.
