OF THESE RESIDENTS, 31% OF THE TENANTS WERE IDENTIFIED AS AT-RISK ELDERLY, AND 11% WERE IDENTIFIED AS FRAIL ELDERLY. HONG LOK HIRED A SERVICE COORDINATOR IN 2012 TO ASSIST RESIDENTS WITH SOCIAL SERVICE NEEDS. APPROXIMATELY 85% OF THE RESIDENTS UTILIZED THIS SERVICE COORDINATOR FOR A VARIETY OF REFERRALS AND ASSISTANCE INCLUDING: BENEFITS/ENTITLEMENTS/INSURANCE, EDUCATION/EMPLOYMENT, HEALTH CARE SERVICES, HOMEMAKER, (CON'T ON SCH O) ISOLATION INTERVENTION, MEALS, MONITORING SERVICES, TRANSLATION/INTERPRETATION AND TRANSPORTATION. BECAUSE THE MAJORITY OF RESIDENTS AT HONG LOK HAVE LIMITED ENGLISH PROFICIENCY AND WHOSE PRIMARY LANGUAGE IS MANDARIN OR CANTONESE, TRANSLATION AND INTERPRETATION SERVICES ARE ESSENTIAL FOR THEIR WELL-BEING. FORTUNATELY, THE SERVICE COORDINATOR, MANAGER AND MAINTENANCE TECHNICIAN ALL SPEAK THESE LANGUAGES, PROVIDING A CRITICAL SERVICE TO THE RESIDENTS. RESIDENTS UTILIZED THE SERVICES AND PARTICIPATED IN PROGRAMS AT THE NEARBY CHINESE SENIOR CITIZENS CENTER. EACH WEEKDAY, THE CENTER PROVIDED TRANSPORTATION TO AND FROM THE APARTMENT COMMUNITY AND THE CENTER. HONG LOK HAS A NEIGHBORHOOD NETWORK CENTER (NNC) WHICH PROVIDED ONSITE COMPUTERS AND HIGH-SPEED INTERNET ACCESS FOR THE RESIDENTS. STAFF ALSO ASSISTED IN FACILITATING PROGRAMS AND ACTIVITIES ON-SITE FOR THE RESIDENTS, INCLUDING: WEEKLY TRANSPORTATION TO RANCH MARKET GROCERY STORES, ST MARY'S FOOD BOX DELIVERY (MONTHLY), BIRTHDAY PARTIES, HOLIDAY CELEBRATIONS, GAMES/MAH JONG, HEALTH FAIRS, BLOOD PRESSURE CHECKS, POTLUCKS, FREE HAIRCUTS, DAILY REJUVENATION EXERCISE/YOGA, KARAOKE, CHOIR AND OPERA. RESIDENT IN-SERVICES WERE CONDUCTED FOR TOPICS INCLUDING: HEALTHCARE ENQUIRY SERVICES AND VARIOUS HEALTHCARE TOPICS, ELDER RIGHTS, XPRESS CARE (FREE MEDICATION DELIVERY), MEDICARE AND COMPREHENSIVE HEALTHCARE. THE RESIDENTS HAVE AN ACTIVE RESIDENT COUNCIL THAT MEETS MONTHLY.
