SENIOR CARE PROGRAM: PROVIDES RESIDENTIAL CARE TO THE ELDERLY WITH SKILLED NURSING, ASSISTED LIVING, AND INDEPENDENT LIVING FACILITIES; PROVIDES SUPPORTIVE SERVICES FOR THE ELDERLY IN THOSE FACILITIES AND OUT IN THE COMMUNITY; PROVIDES HOME HEALTH CARE SERVICES. IN 2018, OUR LONG-TERM CARE FACILITIES SERVED 1,629 PATIENTS; OUR INDEPENDENT LIVING COMMUNITIES SERVED 1,140 RESIDENTS; AND OUR HOME HEALTH CARE PROGRAM SERVED 367 CLIENTS.
