Tri-Valley Care (LA Wraparound) provides wraparound services in Louisiana regions 1 (Jefferson, Orleans, and St. Bernard parishes) and regions 2 (East Baton Rouge, West Baton Rouge, Ascension, Iberville, Pointe Coupee, East Feliciana, and West Feliciana parishes) serving 1,268 youth and their families during the year 2017. A total of 4,938 have been served since the program began in 2012. Wraparound is a process that helps youth with complex needs benefit from a personalized, streamlined care planning process using a single plan of care that includes all formal service providers and natural supports for the family. Wraparound integrates youth into the community and builds the family's social support network, rather than seeking out of home placement. Qualifying youth have significant mental and behavioral health challenges and can receive wraparound services up through the age of 21. Some accomplishments during 2017 include: only 4% of all youth spent time inpatient; 73% of showed improved clinical functioning; 67% showed improved school functioning; 93% of the youth served were discharged to a home/community-based setting.
