HUD THE HUD MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES PROGRAM'S AIM IS TO PROVIDE COMMUNITY OUTREACH EFFORTS IN EL PASO COUNTY TO HELP IDENTIFY AND LOCATE HOMELESS CLIENTS AND FAMILIES IN THE COMMUNITY AND STREETS IN NEED OF BOTH BEHAVIORAL HEALTH AND PRIMARY CARE SERVICES. STAFF WILL COLLABORATE AND ENGAGE KEY COMMUNITY STAKE HOLDERS TO PROMOTE OUR ACCESS TO THE CONTINUUM OF CARE SYSTEM. THIS INCLUDES ENGAGING HOSPITAL EMERGENCY ROOMS AND OTHER KEY HEALTH CARE PROVIDERS TO IDENTIFY HOMELESS CONSUMERS AND TO REFER THEM TO OUR PROGRAM. THE OUTREACH/INTEGRATED CASE MANAGER AND SOCIAL WORKERS ALSO VISIT VARIOUS EMERGENCY, TRANSITIONAL, AND PERMANENT HOUSING PROGRAMS WITHIN THE CITY OF EL PASO TO PROMOTE THE PRIMARY AND MENTAL HEALTH SERVICES OFFERED AT THE HEALTH CENTER. ALSO PERSONS ARE LINKED TO SUBSTANCE ABUSE TREATMENT IF NECESSARY. THE PROGRAM ENDED IN AUGUST 2017. HRSA - CHC AND HC PROGRAMS THE OVERALL GOAL OF THE HRSA HEALTH CARE PROGRAM(S) IS TO MAKE AVAILABLE ACCESSIBLE AFFORDABLE PRIMARY CARE IN A MANNER THAT IS PATIENT CENTRIC, CULTURALLY AND LINGUISTIC APPROPRIATE, ADHERES TO EVIDENCE BASED STANDARDS, AND IMPROVES THE HEALTH STATUS OF COMMUNITIES. THE TARGET POPULATION IS THE UNINSURED, AND MEDICALLY INDIGENT. THIS INCLUDES SERVING SPECIAL POPULATIONS SUCH AS THE IMMIGRANT COMMUNITIES, THE HOMELESS, THOSE LIVING WITH HIV, AND MEMBERS OF THE LGBT COMMUNITY. IN THE CURRENT YEAR, THE CENTER SERVED OVER 16,771 UNDUPLICATED PATIENTS. PRIMARY HEALTH CARE PROGRAM - TEXAS STATE DEPARTMENT OF HEALTH THE OVERALL GOAL IS THE PRIMARY HEALTH CARE PROGRAM IS TO OFFER PRIMARY CARE SERVICES TO RESIDENTS OF TEXAS WHO ARE UNINSURED AND ARE AT OR BELOW 200% OF THE FEDERAL POVERTY LEVEL. IN THE CURRENT YEAR, THE PRIMARY HEALTH CARE PROGRAM SERVED 2,101 PATIENTS.
