CHRISTUS SPOHN HEALTH SYSTEM DEVELOPMENT FOUNDATION (THE "FOUNDATION") IS A TEXAS NON-PROFIT ORGANIZATION SERVING THE PHILANTHROPIC NEEDS OF CHRISTUS SPOHN HEALTH SYSTEM CORPORATION (THE "HEALTH SYSTEM"). IT IS THE FOUNDATION'S RESPONSIBILITY TO ENCOURAGE AND RECEIVE CONTRIBUTIONS FROM INDIVIDUALS, CORPORATIONS, ASSOCIATIONS, AND FOUNDATIONS. THE FOUNDATION PROVIDED FUNDS TO THE HEALTH SYSTEM DURING FISCAL YEAR 2017 FOR CAPITAL EQUIPMENT, MAMMOGRAM SCREENING, BREAST CANCER AWARENESS, SERVICES FOR THE MEDICALLY INDIGENT AND HOSPITAL IMPROVEMENTS. THE FOUNDATION'S SOLE CORPORATE MEMBER IS THE HEALTH SYSTEM; THE HEALTH SYSTEM'S SOLE CORPORATE MEMBER IS CHRISTUS HEALTH. CHRISTUS SPOHN HEALTH SYSTEM IS PART OF CHRISTUS HEALTH, WHICH WAS FORMED IN 1999 TO STRENGTHEN THE 152-YEAR-OLD, FAITH-BASED HEALTH CARE MINISTRIES OF THE CONGREGATIONS OF THE SISTERS OF CHARITY OF THE INCARNATE WORD OF HOUSTON AND SAN ANTONIO.
