NORTHSIDE SHARES HELP, INC. ("SHARES HELP") IS ORGANIZED PRIMARILY TO RECEIVE CONTRIBUTIONS FROM EMPLOYEES AND RETIREES OF NORTHSIDE HOSPITAL, INC. AND TO DISTRIBUTE THE PROCEEDS TO NEEDY NORTHSIDE HOSPITAL EMPLOYEES AND RETIREES. DURING EACH CALENDAR YEAR, DONATIONS ARE DISBURSED FOR THE EMERGENCY AID OF NORTHSIDE HOSPITAL EMPLOYEES AND RETIREES, WHO ARE NATURAL PERSONS, AND THEIR DEPENDENTS.TO QUALIFY FOR EMERGENCY AID, NORTHSIDE HOSPITAL EMPLOYEES OR RETIREES MUST BE IN DIRE FINANCIAL NEED AS A RESULT OF AN EMERGENCY. A NORTHSIDE HOSPITAL EMPLOYEE OR RETIREE IS IN DIRE FINANCIAL NEED WHEN HIS/HER AVAILABLE FINANCIAL RESOURCES ARE INSUFFICIENT TO PROVIDE FOOD, SHELTER, CLOTHING, MEDICAL ATTENTION OR OTHER BASIC NECESSITIES OF LIFE FOR THE NORTHSIDE HOSPITAL EMPLOYEE, RETIREE OR HIS/HER DEPENDENTS.ANY NORTHSIDE HOSPITAL EMPLOYEE OR RETIREE WHO WISHES TO BE CONSIDERED FOR EMERGENCY AID COMPLETES A FORMAL APPLICATION WHICH CONTAINS THE NORTHSIDE HOSPITAL EMPLOYEE OR RETIREE'S FINANCIAL RESOURCES, THE EMERGENCY GIVING RISE TO THE REQUEST FOR AID AND THE AMOUNT OF AID SOUGHT.AFTER THE APPLICATION IS REVIEWED FOR ELIGIBILITY BY THE SHARES HELP COORDINATOR, THE SELECTION COMMITTEE OF SHARES HELP (COMPRISED OF NORTHSIDE EMPLOYEES AND RETIREES) CONSIDERS AND VOTES UPON EACH APPLICATION FOR EMERGENCY AID GREATER THAN $100, WHICH IS NOT TO EXCEED AMOUNTS SPECIFIED IN THE SHARES HELP POLICIES.THE COORDINATOR AND THE SELECTION COMMITTEE OF SHARES HELP MAY UNDERTAKE SUCH INVESTIGATION AS IT SHALL DEEM NECESSARY OR PROPER TO VERIFY THE FACTS SET FORTH IN AN APPLICATION BY A NORTHSIDE HOSPITAL EMPLOYEE OR RETIREE FOR EMERGENCY AID. ALL DECISIONS MADE BY THE SELECTION COMMITTEE ARE FINAL.
