About

City Health Works continued delivery of health coaching and expanded services across upper Manhattan with the launch of our first site of operation in a new geography - in Schenectady, NY for patients with diabetes, hypertension, and asthma. We have continued delivery of services for patients with poorly controlled diabetes, hypertension and asthma across Harlem with additional funding support through the Mount Sinai DSRIP program for patients referred from clinics across Harlem. City Health Works continued to strengthen its operational systems. It completed the vendor approval process for Healthfirst by achieving the regulatory requirements for HIPAA required of covered entities. Our software continued to undergo several advancements, particularly in the areas of tools for health coach supervisors and features to support addition of a new geographic site. In addition to continued work on various research projects, City Health Works received press coverage and presented at several events.
