About

Concordia Visiting Nurses (CVN) program service accomplishments are driven by the core mission to provide excellent in-home health services to those who need them, in a faithful environment. CVN is part of a large continuum of care striving to succeed with an aging in place philosophy by providing a wide breadth of services in the home, preventing rehospitalization, enhancing home safety and monitoring chronic disease states. The organization is committed to providing the highest quality of care from nursing and therapy to specialized services such as IV therapy, wound care, palliative care, and psychiatric care, all from the comfort of the patient's home. Approximately 15,483 patients were served across twelve counties in Western Pennsylvania with an average daily census of 2,036. A total of approximately 266,754 visits (including 4,542 non billable visits associated with spiritual care, nutrition, etc.) were completed by various disciplines. Additionally, as part of the prevention efforts for the deterioration of existing health conditions, CVN provides Tele-Health monitoring free of charge to patients with chronic diseases like congestive heart failure. Tele-Health monitoring is a free service to the community but cost the agency $345,709 annually. CVN serves residents in the following counties; Allegheny, Armstrong, Beaver, Butler, Greene, Lawrence, Washington and parts of Clarion, Indiana, Fayette, Mercer and Westmoreland counties. CVN employs a Community Educator and a team of Transitional Nurse Liaisons who serve the community by providing a variety of information, screenings, speaking topics on healthy living and disease prevention. They also team up with CVN nurses, social workers, and physical therapists to provide balance screening assessments, Health Chats and "Ask The Nurses" programs. Each month our Community Educator and our Transitional Nurse Liaisons schedule and attend several community gatherings to discuss our "Concordia Visiting Nurses Healthy Topic" of the month at health fairs, community speaker programs and events. Our goal is to reach as many people as possible in the community to promote wellness and teach prevention. Ideally, we would like to prevent the deterioration of existing health conditions and identify and teach early intervention for exacerbations of diseases and careful management to prevent hospitalizations. In providing screenings, we are attempting to be proactive in the early diagnosis and treatment of chronic disease conditions. Any senior adult with questionable screening results or symptoms are always referred to their physician for further testing and treatment and timely follow-up occurs with the senior by a member of our team. We are receptive to and always looking for ways to enhance Concordia's exposure throughout the community. These free education and screening events are conducted at a variety of apartments, housing centers, senior centers, events at local malls, churches, civic centers, and other health care facilities. The following, but not limited to, are the venues attended in the Cabot, Baden, New Castle and Monroeville service areas on a monthly, quarterly or yearly basis for Blood Pressure Clinics, Health Chats and Health Fairs etc for 2017-2018: New Bethlehem Broadwood Towers and the New Bethlehem Highrise, New Bethlehem, PA; Allegheny Hills Retirement Center, East Brady, PA; Southside Senior Center, Shippingport, PA; Conway Towers, Baden, PA; Economy Senior Center, Economy, PA; St Francis Gabini 50+Women, Aliquippa, PA; Senior Day at The Mall, Center Township Apartments, Monaca Senior Center, The Veterans Chili Cookoff at the Center of the Mall Event, in Monaca, PA; New Brighton Life Center and the Coffee Klatch in New Brighton, PA; Lutheran Village Seniors, Beaver Falls, PA; Challenges Senior Center AAA, Neshannock Woods Apartments, and the Challenges Senior Center Fitness Walk 5K in New Castle, PA; Concordia Havens I, II, III, Concordia Adult Day Care and the Winfield Senior Center, Cabot, PA; Saxonburg Commons, Saxonburg, PA; Butler Senior Center, Butler, PA; Concordia of the South Hills, Pittsburgh, PA. Additional Health Fairs attended include: Senator Vogel Health Fair in New Castle, PA and Beaver, PA; Representative Marshall Health Fair in Beaver Falls, PA; Circle of Friends Health Fair; Slippery Rock Senior Center Health Fair, Tanglewood Senior Center Health Fair; and the Butler County Housing Authority Fall Health Fairs which included 1 fair at each of the HUD high rises for Butler County. Members attended Hazelwood Community Day, attended the Fair at Villa St Joseph and promoted telehealth and therapy services, hosted a Continuing Education Course at Butler Community College for therapy, attended the Alzheimer's Walk and supporting events in Washington, Beaver and Butler counties, sponsorship of the Veterans Breakfast Club and supporters of the New Haven Court Community Blood Drive. Blood Pressure Screenings were attended at Springdale Senior Center; Congestive Heart Failure Educational Series at Butler Memorial Hospital; Alexis Manor, Wexford, PA; Chicora Senior Center; Slippery Rock Senior Center; Plum Senior Center; Mt. Lebanon Senior Center; Park Manor, Penn Hills, PA; and Sunbury Fields, Butler, PA. 518 flu shots were provided by CVN at Flu Clinics conducted in 2017-2018 which were held at Octoberfest at the Concordia Haven Apartments, Baglier Buick, Ron Gross Trucking, XTO Energy in Warrendale to offer Flu Shots to members of the community. Many employees of CVN volunteer their time to serve at the Annual Summer Festival and OctoberFest, both of which are sponsored by Concordia Lutheran Health and Human Care and are open to the public as a way of providing entertainment, food and refreshments and a feeling of community. One CVN employee in particular devotes an extra 40 hours per year at a cost of $790 to coordinate the Kids Area for each festival which includes activities, crafts and face painting, all of which are provided free to the public. We also have an employee who volunteers to take an annual trip to an orphanage in Guatemala to deliver medical supplies, personal care items, clothing, and toys to those in need. Some of the supplies are expired medical items donated by CVN and others were gathered and donated by fellow CVN employees. Other community education that was provided was an event at Parker Senior Center (Parker, PA) to educate on Aging & Memory and Compensatory Strategies. This was presented by the Therapy Manager and Marketing Liaison, who were also able to renew CVN services for patients and caregivers at the facility while there. In FYE 2018, Concordia Visiting Nurses donated the use of the board room and equipment to St Luke Lutheran Church to interview prospective pastoral call candidates. There were three meetings at three hours each with a total cost to the agency of $300. CVN hosted 8 students from Concordia University of Ann Arbor (Ann Arbor, MI) for an opportunity for the students to travel with CVN nurses to see patients. Two students per day completed hands-on care as well as observation of the staff over a course of four days with a total cost to the agency of about $2,970. The following, but not limited to, have received direct financial support from CVN through donations and sponsorships of events: American Cancer Society Nick Stupakis Real Men Wear Pink fundraiser, Armstrong County Memorial Hospital Foundation Golf Outing, Armstrong County Memorial Hospital Foundation Wings of Hope Gala, Beaver County Christian School, Beaver County Senior Olympics, Butler Community Health Clinic fundraiser, Butler Health System Foundation Gala, Concordia Lutheran Ministries Golf Outing, Concordia of the South Hills Holiday Bazaar, Economy Volunteer Fire Dept, Heritage Valley Beaver Holiday Ball, Heritage Valley Health System Golf Outing, and the Washington Health System Foundation Clay Shoot. Total donations and sponsorships made during fiscal year ended 6/30/18 equated $27,280. Additional funding via distributions to 501(c)(3) member hospital systems at a total of $4,000,000 have expanded like-minded mission within the geographical communities we serve. Employees of CVN were given the opportunity to participate in several Mission Events organized by the parent company Concordia Community Support Services, including a toy drive to collect Christmas gifts for The Lighthouse Foundation's Toy Shoppe and serving a shift at The Lighthouse Foundation Food Bank (Butler, PA). These are part of a new Mission Committee that has been formed to organize opportunities for employees to serve the community.
