HEARTH CONNECTION WORKS WITH A BROAD SPECTRUM OF PARTNERS WITH THE GOAL OF ENDING LONG-TERM HOMELESSNESS AND CURRENTLY OFFERS SUPPORTIVE HOUSING SERVICES IN 46 COUNTIES IN MINNESOTA. AS AN INTERMEDIARY BETWEEN GOVERNMENT ENTITIES AND DIRECT SERVICE PROVIDERS, HEARTH CONNECTION COLLABORATES WITH PUBLIC AND COMMUNITY PARTNERS TO EXPAND SUPPORTIVE HOUSING OPPORTUNITIES, IMPROVE PARTICIPANT OUTCOMES AND ENSURE PARTICIPANTS CAN THRIVE AND PROSPER. IN 2018 HEARTH CONNECTION MANAGED FINANCES, ADMINISTRATION, SERVICE PROVIDER OVERSIGHT, REPORTING AND OTHER FUNCTIONS FOR FOUR (4) REGIONAL SUPPORTIVE HOUSING COLLABORATIVES AND COUNTY PROJECT THAT SERVED OVER 2,368 PROGRAM PARTICIPANTS IN OVER 1,349 HOUSEHOLDS IN THE METRO, NORTHEASTERN, SOUTHEASTERN AND CENTRAL MINNESOTA AND DAKOTA COUNTY PROJECTS.IN ADDITION TO THESE REGIONAL PROJECTS, HEARTH CONNECTION OPERATED THE FOLLOWING PROGRAMS IN 2018: THE STEP-DOWN PILOT PROJECT, WHICH HELPS MOVE METRO-AREA LONG-TERM HOMELESS PEOPLE FROM SITE-BASED SUPPORTIVE HOUSING INTO SCATTERED-SITE HOUSING; THE 811 PILOT PROJECT, WHICH PROVIDES LONG-TERM SUPPORTIVE SERVICES TO INDIVIDUALS EXPERIENCING SERIOUS MENTAL ILLNESS AND LONG-TERM HOMELESSNESS WHO RESIDE IN SECTION 811 UNITS; THE DAKOTA COUNTY HOUSING SEARCH AND STABILITY SERVICES PROGRAM, WHICH WORKS WITH PARTICIPANTS TO LOCATE AND OBTAIN HOUSING WITH RENTAL ASSISTANCE PROVIDED BY DAKOTA COUNTY OR THE DAKOTA COUNTY COMMUNITY DEVELOPMENT AGENCY; COORDINATED ENTRY NAVIGATION SERVICES, WHICH ALLOWS FOR HOUSEHOLDS EXPERIENCING HIGH BARRIERS TO HAVE ACCESS TO SUPPORT SERVICES WHILE WAITING FOR AN OPENING IN A LOCAL HOUSING PROGRAM; AND THE COMING HOME PILOT PROJECTS, WHICH CONNECT INDIVIDUALS WITH MENTAL ILLNESS AND LONG-TERM HOMELESSNESS WHO ARE FREQUENT USERS OF EMERGENCY HEALTHCARE SERVICES TO SUPPORTIVE SERVICES AND SHORT AND LONG-TERM HOUSING OPPORTUNITIES. HEARTH CONNECTION ALSO PROVIDES RENTAL ASSISTANCE TO PARTNER PROVIDERS WHO ARE ALREADY SERVING PEOPLE EXPERIENCING CHRONIC HOMELESSNESS AND WHO HAVE VARIED FUNDING SOURCES BUT STILL NEED ADDITIONAL RENTAL ASSISTANCE FUNDS.HEARTH CONNECTION IS COMMITTED TO THE INNOVATIVE USE OF DATA, EVALUATION AND RESEARCH TO PROVIDE ACCOUNTABILITY AND IMPROVE SERVICES. IN ADDITION TO DATA COLLECTED IN CO-PILOT, HEARTH CONNECTION'S CUSTOMIZED DATABASE, THE NONPROFIT ALSO USES THE ARIZONA SELF-SUFFICIENCY MATRIX TO TRACK PARTICIPANTS' PROGRESS IN 18 DIFFERENT AREAS OF FUNCTIONALITY INCLUDING PARTICIPANT PRODUCTIVITY AND EMPLOYMENT. HEARTH CONNECTION ALSO RELIES ON DATA TO DEMONSTRATE THE VALUE OF HOUSING AND SUPPORTIVE SERVICES TO STATE AND IN 2018 CONTINUED WORKING WITH ADVOCATES AND LEGISLATORS TO INCREASE STATE RESOURCES FOR LONG-TERM HOMELESS SERVICES. RECENT DATA SHOW THAT 71 PERCENT OF REGIONAL PROJECT PARTICIPANTS SECURE PERMANENT HOUSING WITHIN THREE MONTHS OF ENROLLMENT AND OVER TWO-THIRDS OF PARTICIPANTS HAVE BEEN STABLY HOUSED FOR AT LEAST ONE YEAR. MOVING FORWARD, HEARTH CONNECTION IS COMMITTED TO JOINING WITH PARTNERS, SERVICE PROVIDERS, STAFF, PARTICIPANTS, AND ADVOCATES IN A COLLECTIVE EFFORT TO END HOMELESSNESS IN MINNESOTA.
