The project is a collaborative effort of the regional Northwest Michigan chronic disease prevention coalition, which serves as the foundation for the network, and the local health coalitions across the region, which serve as the local implementation partners. This project focuses on strategies that link directly to the DHD#10 chip specifically to focus areas: promotion of healthy lifestyles and behaviors and creation and maintenance of healthy communities targeting at-risk populations and medically underserved populations.
