HOSPICE CARE PLUS, INC. IS A NONPROFIT HEALTHCARE ORGANIZATION SERVING TERMINALLY ILL PATIENTS IN MADISON, ROCKCASTLE, ESTILLE, LEE, OWSLEY AND JACKSON COUNTIES OF KENTUCKY. THE MISSION OF HOSPICE CARE PLUS, INC. IS TO PROVIDE COMPASSIONATE, SUPPORTIVE CARE TO PERSONS WITH A LIFE-LIMITING ILLNESS, HONORING THEIR VALUES, BELIEFS, AND NEEDS AND THOSE OF THEIR FAMILIES AND THE COMMUNITIES SERVED. THE SERVICES INCLUDE A 24-HOUR NURSING SUPPORT FOR PAIN AND SYMPTOM MANAGEMENT, SOCIAL WORKERS TO COORDINATE COMMUNITY RESOURCES, PROVIDE EMOTIONAL SUPPORT AND ASSISTANCE WITH INSURANCE AND BEREAVEMENT CARE, CHAPLAINS TO ASSIST THE PATIENTS AND FAMILIES WITH SPIRITUAL CONCERNS, NURSING ASSISTANTS FOR PERSONAL CARE NEEDS AND LIGHT HOUSEKEEPING, A PHARMACIST TO HELP ADDRESS MEDICATION NEEDS, MEDICATIONS, MEDICAL EQUIPMENT AND SUPPLIES DELIVERED TO PATIENT'S HOME AND OVERSIGHT OF CARE BY PATIENT'S PHYSICIAN AND A HOSPICE MEDICAL DIRECTOR. SERVICES ARE PROVIDED IN TEH HOME SETTING, HOSPITAL, NURSING HOME OR INPATIENT UNIT. DURING FISCAL YEAR 2018, HOSPICE CARE PLUS, INC. SERVED AN AVERAGE OF 95 PATIENTS PER DAY IN THE HOME PROGRAM AND 7 PATIENTS PER DAY IN THE INPATIENT PROGRAM.
