Seven Counties Services is a community mental health center that offers a full range of mental and behavioral health services, substance abuse treatment, and intellectual and developmental disabilities services in a seven-county region in Kentucky. Counties served include Jefferson, Oldham, Bullitt, Shelby, Spencer, Trimble, and Henry.

Community-based adult services assertive community treatment, intensive assertive community engagement, adult services, engagement, homeless outreach, criminal justice programs, therapeutic rehab services, case management, housing, and supported employment. Centerstone provides community-based treatment for our most vulnerable citizens with severe and persistent mental illness (SPMI) and/or co-occurring substance abuse use disorders. We have a dedicated team of staff who provide a range of services to maximize the client's level of independence, life functioning, and quality of life. The individual, in partnership with their team, plan, coordinate, monitor, adjust, and advocate for services and supports directed toward the achievement of the individual's personal goals for community living. In FY 18-19, community-based adult services served 1,271 clients.
