About
Conflict free case management continues to be our only program. The way this is delivered was changed by NYS as of July 1, 2018. The case management that Link does was changed from Medicaid service coordination to health home/care coordination. We retained our same employees and serve the same clients. In 2018 we went from serving 425 individuals who received home and community-based waiver services to serving 335 individuals. The health home/care coordination model requires smaller case loads, and intake of new clients is handled by the care coordination entity, Prime Care Coordination, Inc. Our staff has decreased from nine full-time and sixteen part-time care coordinators to nine full-time and eight part-timers, and from three part-time administrative assistants to two. Link continues to help individuals with developmental disabilities to find services, connect to services, and maintain their chosen services and supports.
