VNA Alliance is an independent, voluntary, nonprofit home health agency licensed in Pennsylvania. All services are available without regard to race, religion, creed, color, age, ancestry, national origin, sex, sexual preference, lifestyle, socioeconomic status, veteran's status or handicap. The agency strives to operate in a Christian manner in all interactions with consumers, staff, and the community. The provision of care to consumers recognizes and supports consumers' rights and the inherent worth and dignity of each individual. The agency seeks to provide care to all consumers in need of its services while recognizing its goal to maintain financial viability. Service delivery is based on the belief that quality health care can be delivered using efficient, effective and ethical business practices. Approximately 2,761 persons were served with an average daily census of 287 patients. A total of approximately 40,876 visits (not including 120 non-billable visits) were completed by various disciplines. For fiscal year ending 06/30/2019, a total of $2,925 in services was rendered as benevolent care. VNA Alliance continues to provide a wealth of community services to the residents, organizations and various groups in Mercer, Crawford, Venango and parts of Erie, Lawrence and Butler Counties. Care is provided in a holistic, comprehensive manner that helps consumers achieve their desired quality of life and functional outcomes. Service to the community is achieved through responsive consumer care, caregiver support and education. Working as an interdisciplinary team, staff respect and support each other. Continuous quality improvement is integral to all agency functions with emphasis on employee development, risk management, strategic planning and organizational performance. Throughout the fiscal year, quality initiatives directed our care delivery. Focused attention on high risk, high volume populations such as CHF and COPD fostered stabilization percentages for the patients that we serve which exceeded national averages by 8.24% and 5.44% respectively. The VNA Alliance's robust disaster preparedness program ensured that close to three thousand residents of our local communities had tailored instructions to follow in the event of a natural disaster. Each year across the country, adverse drug events account for over 700,000 ED visits and over 100,000 hospitalizations. To address this national crisis within our community, the VNA Alliance has actively pursued establishing medication safety and self-efficacy within our patient population. Over the past year, the VNA Alliance objectively improved our patient's safe ability to manage their oral medications by over 24 percent. For the past fiscal year, the VNA Alliance has been ranked in the top 15% of all home health care agencies nationally in areas related to improvements in mobility, activities of daily living, return trips to the hospital and timely initiation of care. Caregiver assistance and support continues to be a top priority for the VNA Alliance. We have provided one on one and group educational trainings for those providing services for the geriatric and complex patient population, such as those with dementia or Alzheimer's disease. Multiple educational sessions pertaining to ergonomics, transfer mechanics, adaptive equipment, restorative and wellness activity prescriptions have been held throughout our coverage area. Lastly, over the past year, the VNA Alliance has provided four separate flu clinics, located across our coverage area and in collaboration with our partners, to foster proactive health promotion habits that have been proven to mitigate infectious risks within our elderly and frail population. 180 doses of flu vaccine were administered this year to employees and at flu clinics. We have gained recognition as an agency that is respected as a contributing member of the community, truly making a difference in the lives of those residing in our geographical boundaries. The focus of our community engagement is geared primarily towards educating and screening the retirement and low-income populations, as these groups seem to have the greatest need regarding health screening and health education. We work closely with individuals who provide direct hands-on care or oversee the health care of these populations as well as general potential consumers of health care services in our communities. To aide in our ability to successfully communicate with our patient population, our liaisons and clinical team utilize common educational materials regarding high risk, high volume and problem prone conditions such as CHF and COPD. This material harmonizes with Meadville Medical Center and Grove City Medical Center so key themes of disease management can travel across care settings. We have continued the tradition of cross training with local educational LPN programs within Crawford and Mercer Counties to provide real time opportunities for nursing students to grow in their knowledge and proficiency under the guidance of seasoned VNA Alliance nurses. The VNA Alliance marketing team maintains a steady presence in the community that we serve. This includes organizing and completing weekly meetings, facilitating discharge planning, and coordinating skilled therapy assessments for the residents of nearby facilities. The VNA Alliance is recognized as a wealth of information for local skilled nursing facilities, assisted living facilities and personal care homes in the community, frequently advising and assisting them to meet the needs of their residents. The vision statement of VNA Alliance is to serve the triune God and the needs of His people. Employees of VNA Alliance were encouraged to participate in several mission events throughout the year in conjunction with Concordia Community Support Services Mission Committee. Thanksgiving Blessing Bags were delivered to homebound patients in November 2018. The spirit of missions was contagious during the Christmas season, when field and office staff members alike went above and beyond; organizing a food drive to support our local food pantry. In March 2019, several No-Sew blankets were constructed and delivered to the Ronald McDonald house in Pittsburgh to bless families who are in need of comfort. In October of 2018, the VNA Alliance took part in the Meadville Chamber of Commerce Halloween Parade, which is Pennsylvania's largest night time parade theme. Our agency attended as "Little Red", "Granny" and of course the "Big Bad Wolf". There were several donations of candy from the agency and staff and all added to the delight of the children along the parade route. VNA Alliance has sponsored several events in the community during the tax year including Charity Golf Outings for: Meadville Medical Center Foundation, Active Aging Foundation, Kiwanis Club of Greenville, and Wesbury Foundation. We were the lead sponsor of the Meadville Medical Center Foundation Stroke Walk. And tickets and sponsorships to other events including St Paul Homes Caring Hearts on Fire Gala, Wesley Family Services Spring Gala, Wesbury Foundation Tribute Banquet, and Greenville Chamber of Commerce Annual Dinner. These donations and sponsorships amount to a cost to the agency of $5,573 and benefit the community that we serve.
