Our mission is to improve the health of our patients and the community and advocate the feminist goals of social, political, and economic equality. Driven by a 40-year commitment to health care as a human right, Santa Cruz Community Health Centers is a leading nonprofit provider of high-quality, affordable and comprehensive health services in English and Spanish to all ages, gender identities, ethnicities, abilities and sexual orientations, regardless of their ability to pay. We operate two clinics: the Santa Cruz Women's Health Center and the East Cliff Family Health Center.

IN FISCAL YEAR ENDED 2019, THE SANTA CRUZ COMMUNITY HEALTH CENTERS SERVED 11,403 PATIENTS VIA 53,368 VISITS. THIS WAS AN 11% INCREASE IN PATIENTS SERVED AND AN 8% INCREASE IN OVERALL VISITS. WE SAW A 32% INCREASE IN THE NUMBER OF HOMELESS PATIENTS SERVED: 929 PATIENTS COMPARED TO 700 LAST YEAR.SCCHC CONTINUED TO STRENGTHEN THE PATIENT-CENTERED MEDICAL HOME MODEL OF CARE, STRENGTHENING EFFORTS TO ADVANCE TEAM-BASED CARE AND INTEGRATED BEHAVIORAL HEALTH. SCCHC QUALITY OF CARE OUTCOMES CONTINUED TO BE VERY STRONG; BASED ON THE CENTRAL CALIFORNIA ALLIANCE FOR HEALTH'S THIRD-PARTY REVIEW, SCCHC SIGNIFICANTLY EXCEEDED IT'S PEERS IN TERMS OF PRIMARY CARE QUALITY SCORES.THIS YEAR THE SCCHC BOARD MADE A COMMITMENT TO EXPAND ITS LIVE OAK SERVICES BY BUILDING A NEW SITE IN PARTNERSHIP WITH DIENTES COMMUNITY DENTAL CARE ("DIENTES"); AND MIDPEN, AN AFFORDABLE HOUSING DEVELOPER.
