During 2017, Rehab requested and received approval from NYSDOH to take over the operation of the four adult healthcare programs run by H.F.C commencing on September 28, 2017. H.F.C's operations are included in Rehab for 2018. Guildcare enables people with chronic health conditions and functional impairments to delay institutionalization while receiving medical services and health monitoring. In 2018, members received services such as nursing, physical, occupational, and speech therapy, vision rehabilitation, social work and nutrition counseling. They benefit from activities designed to maximize their function and independence, and from health education focused on medication and dietary compliance to prevent diabetic complications, including vision loss. Guildcare also provides meals and door-to-door transportation.
