Care Coordination- Partners in Care Foundation's short-term services and supports help keep high risk patients stabilized at home. Our Care Transitions program works with physicians, hospitals, health plans and skilled nursing facilities, by providing health coaches to help patients make their best possible recovery after hospitalization. That includes coaching patients to make personal plans for better health, and connecting them to community-based services that stabilize them at home and avoid hospital readmission. Our complex care coordination provides health coaching assistance for up to 90 days for people with the most complicated health challenges. Our HomeMeds program inventories medications and coordinates with a pharmacist, the patient and their doctor to identify and resolve medication-related risks.
