Our Mission is to build welcoming communities and caring relationships that empower people and restore hope. Our vision is that people empowered by community and relationships find strength and hope, heal and recover and lighten for each other the burdens of human experience in all aspects of their lives. Our purpose is to provide effective, community-based resources to vulnerable adults living with serious mental illness in San Francisco. Those resources - housing, counseling and jobs help residents and clients find healthy pathways to recovery and self-manage chronic medical and mental illnesses. Each year, Conard House serves more than 1,500 adults through its welcoming, caring and empowering communities.

DPH SUPPORTIVE HOUSING PROGRAM - THIS PROGRAM PROVIDES LONG-TERM AND TRANSITIONAL SUPPORTIVE HOUSING FOR ADULTS. SERVICES INCLUDE MENTAL HEALTH OUTPATIENT SERVICES, CASE MANAGEMENT, MONEY MANAGEMENT, AND OTHER SUPPORT SERVICES AT THE FOLLOWING SAN FRANCISCO LOCATIONS: THE EL DORADO, THE WASHBURN RESIDENCE, THE PLAZA APARTMENTS, THE LYRIC, THE MIDORI, AND THE JORDAN APARTMENTS. THE WASHBURN RESIDENCE OFFERS 24-MONTH TRANSITIONAL HOUSING WHILE ALL OTHERS OFFER LONG-TERM HOUSING. THIS PROGRAM ALSO INCLUDES APPROXIMATELY 17 APARTMENTS WHICH PROVIDE SUPPORTED INDEPENDENT LIVING WITH MENTAL HEALTH OUTPATIENT SERVICES IN COOPERATIVE (SHARED) HOUSEHOLDS TO GROUPS OF FOUR TO SIX ADULTS IN ITS LEASED APARTMENTS AND ITS OWNED FACILITIES AT 26TH STREET AND MCALLISTER STREET.
