THE ORGANIZATION PROVIDED DECENT, SAFE, SANITARY AND AFFORDABLE HOUSING TO 48 RESIDENTS: 7% BETWEEN THE AGES 18-61 (NON-ELDERLY PEOPLE WITH DISABILITIES), 79% BETWEEN THE AGES 62-80, 14% BETWEEN THE AGES 81-95 AND ZERO 96+. OF THESE RESIDENTS, APPROXIMATELY 27% WERE ASSESSED TO BE AT-RISK ELDERS AND 16% WERE ASSESSED TO BE FRAIL ELDERS. ARIZONA RETIREMENT HOMES EMPLOYS A SERVICE COORDINATOR WHO ASSISTED RESIDENTS WITH OBTAINING A VARIETY OF SOCIAL SERVICES AND REFERRALS. THESE SERVICES INCLUDED: ADVOCACY, CASE MANAGEMENT, BENEFITS/ENTITLEMENTS/INSURANCE, CONFLICT RESOLUTION, CRISIS INTERVENTION/SUPPORT COUNSELING, GENERAL (CON'T ON SCH O) INFO/REFERRALS, HEALTHCARE SERVICES, HOME MANAGEMENT, ISOLATION INTERVENTION, FOOD PROGRAMS/MEALS AND OUTREACH. THE SERVICE COORDINATOR ALSO ARRANGED FOR RESIDENT IN-SERVICES AND CLASSES ON TOPICS SUCH AS: TECHNOLOGY, DIABETES EDUCATION AND EMPOWERMENT, MEDICARE Q & A, MEDICARE UPDATES, KINDNESS AND GRATITUDE, HEALTHY HABITS, KEEPING THE FLU AWAY, STROKE AWARENESS (FAST), AND MATTER OF BALANCE: FALL PREVENTION. THE SERVICE COORDINATOR COORDINATED OTHER ON-SITE PROGRAMS FOR: SECOND SATURDAY VOLUNTEERS, BROWN BAG, WASTE NOT, FOOD PLUS, ST MARY'S FOOD BANK, FLU SHOTS, MONTHLY BLOOD PRESSURE CHECKS AND WALKING CLUB/FITNESS/WELLNESS EDUCATION. THE RESIDENTS BENEFITED FROM OTHER WEEKLY AND MONTHLY SOCIAL ACTIVITIES ARRANGED BY THE SERVICE COORDINATOR AND THE RESIDENT COUNCIL, INCLUDING CRAFT CLASS, CARD GAMES, BINGO BIRTHDAY/HOLIDAY CELEBRATIONS AND COFFEE. RESIDENTS ALSO USED THE NEARBY GRANITE REEF SENIOR CENTER FOR ADDITIONAL SERVICES, PROGRAMS AND ACTIVITIES. ARIZONA RETIREMENT HOMES-I SHARES A NEIGHBORHOOD NETWORK CENTER (NNC) WITH ARIZONA RETIREMENT HOMES-II WHICH PROVIDED RESIDENTS WITH THREE COMPUTERS, COMPUTER INSTRUCTION AND HIGH-SPEED INTERNET ACCESS.
