Peerlink Intensive Care Services are designed to connect people with mental health conditions and/or substance use disorders following psychiatric hospitalization and/or being in a crisis stabilization unit to appropriate community-based services, supports, and facilities that can meet the patient's post discharge clinical, social and environmental needs. Each program enrollee is assigned a care specialist that provides support for 90 days and develops a specialized care plan to address ongoing needs so the participant can obtain a better quality of life. This program assists 5,000 individuals annually.
