During 2016, while operating two group homes and four apartments with 21 available beds, Shelter Resources housed 28 permanent residents and 18 transitional residents. All goals for providing services to these individuals living with HIV/AIDS were met or exceeded. Among its accomplishments in 2016:All 46 clients participated in the Outreach Program and received medication adherence counseling.80 individuals received Harm Reduction substance abuse counseling.6 clients were discharged from transitional housing into permanent stable housing.25 clients were discharged from the program during the year having has a visit with a primary medical provider at least once in the 3 months prior to discharge.40 clients successfully accessed or maintained qualification for sources of income.100% of clients achieved increased knowledge of substance abuse and HIV/AIDS.
