THE DISABILITY SERVICES PROGRAM DEVELOPS HOST HOMES WHERE SUPPORT SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES ARE PROVIDED IN A FAMILY ENVIRONMENT. LSG COLLABORATES WITH SUPPORT COORDINATION, DEPARTMENT OF BEHAVIORAL HEALTH AND DEVELOPMENTAL DISABILITIES, STATE HOSPITALS, DFCS, FAMILIES, ADVOCATES AND OTHER STAKEHOLDERS THROUGH A REFERRAL PROCESS. LSG PROVIDES CASE MANAGEMENT AND ENSURES COMPLIANCE WITH MEDICAID GUIDELINES FOR SERVICE PROVISION. IN FISCAL YEAR 2018, 98 INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES WERE MATCHED IN HOST HOMES.
