EFFECTIVE CARE NAVVIS WAS INVOLVED IN THE SUPPORT AND DEVELOPMENT OF THE FOLLOWING STRATEGIC INITIATIVES FOR 2019: COMPLEX PATIENT CENTERED MEDICAL HOME (C-PCMH), COREO CARE, QA COMMITTEE, COMMUNITY ACTION NETWORK (CAN). THE C-PCMH INITIATIVE: IN SUMMARY, THE FOLLOWING WAS ACCOMPLISHED: A. IN ADDITION TO THE CORE CHW WORKER, AN APRN WAS HIRED BY HMC TO PARTNER WITH THE CHW TO MANAGE PATIENTS. BEHAVIORIAL HEALTH AND OTHER 'WRAP AROUND' SERVICES ARE ALSO PROVIDED BY HMC AS NEEDED. B. AS OF FEBRUARY 2019, 85 HMSA QUEST MEMBERS HAVE BEEN CARED FOR THROUGH THE C-PCMH PROGRAM. THE DATA SHOWS A DOWNWARD TREND IN AVOIDABLE ED COSTS/PMPM, ED UTILIZATION, AND HOSPITAL UTILIZATION. COREO CARE INITIATIVE: IS A CARE COORDINATION PLATFORM SUPPORTING POPULATION HEALTH, CASE/DISEASE MANAGEMENT PROGRAMS, UM, AND MEDICAL MANAGEMENT. TO CREATE EFFICIENCY AND ACCOUNTABILITY, THE PILOT FOR AN ONLINE REFERRAL SYSTEM IS CURRENTLY BEING IMPLEMENTED BETWEEN THE HMC C-PCMH, THE BIG ISLAND SUBSTANCE ABUSE COUNCIL (BISAC), HAWAII COUNTY OFFICE OF AGING, HOPE SERVICES FOR THE HOMELESS, AND HUI MALAMA OLA NA 'OIWI. THE GOAL OF THE PLATFORM IS TO COORDINATE CARE AND CONNECT MEDICAL AND SOCIAL SERVICE PROVIDERS IN EAST HAWAII EFFICIENTLY AND EFFECTIVELY. QA COMMITTEE INITIATIVE: IN SUMMARY, THE FOLLOWING WAS ACCOMPLISHED: A. TO ALLOW FOR THE EXCHANGE OF PHI, AN ORGANIZED HEALTHCARE ARRANGEMENT (OHCA) WAS FORMED. B. THE FIRST EAST HAWAII COMMUNITY QA COMMITTEE WAS HELD ON AUGUST 29, 2019 AND INCLUDED 9 MEDICAL AND SOCIAL SERVICE ORGANIZATIONS ALONG WITH HMSA. TWO ADDITIONAL MEETINGS WERE HELD IN 2019 COVERING FIVE COMPLEX, CROSS-SECTOR CASES. PARTICIPANTS INCLUDE BAY CLINIC, HAWAII COUNTY FIRE DEPARTMENT, HILO MEDICAL CENTER, HOPE SERVICES, HMSA, OHANA, ALOHACARE, KA'U RURAL HEALTH COMMUNITY ASSOC., BIG ISLAND SUBSTANCE COUNCIL, HAWAII CARE CHOICES, HAWAII COUNTY OFFICE OF AGING, HUI MALAMA OLA NA 'OIWI, NAVVIS, AND COMMUNITY FIRST. SINCE AUGUST AN ADDITIONAL 2 MEETINGS HAVE BEEN HELD WITH A PLAN TO CONTINUE MEETING AT LEAST QUARTERLY GOING FORWARD. CAN INITIATIVE: IN SUMMARY, THE FOLLOWING HAS BEEN ACCOMPLISHED: A. NAVVIS PROVIDED STAFF AND CONSULTING SERVICES TO SUPPORT CAN. THEY DEVELOP AGENDAS, MATERIALS AND FACILIATED MONTHLY CAN MEETINGS IN CONJUNCTION WITH THE CHAIR (DARRYL OLIVEIRA) AND CO-CHAIR (MARTHA YAMADA). THERE HAS BEEN AN AVERAGE ATTENDANCE OF 35 COMMUNITY MEMBERS, REPRESENTING OVER 20 ORGANIZATIONS. HMSA COMMUNITY ENGAGEMENT COMMIITTEE MEMBERS (TERRY GEORGE AND LORRAINE AKIBA) ALONG WITH SEVERAL HMSA EXECUTIVE LEADERS ATTENDED A CAN MEETING ON JANUARY 15TH, 2020 TO LEARN MORE ABOUT THE GROUP, ITS PARTICIPANTS, AND THE TOPICS DISCUSSED. B. BEHAVIORAL HEALTH: IN RESPONSE TO THE BEHAVIORAL HEALTH CHALLENGES IN EAST HAWAII, THE CAN CREATED A TASK FORCE TO FOCUS ON BEHAVIORAL HEALTH. THE BEHAVIORAL HEALTH TASK FORCE PRODUCED A COMPREHENSIVE BEHAVIORAL HEALTH ARRAY OF SERVICES TO SERVE AS A RESOURCE DIRECTORY FOR EAST HAWAII. ALSO, IN ORDER TO SUPPORT THE COMMUNITY IN ADDRESSING AN IDENTIFIED GAP IN CERTAIN BEHAVIORAL HEALTH RESIDENTIAL SERVICES ON THE ISLAND, NAVVIS SUPPORTED BISAC IN RESPONDING TO THE SUACUTE STABILIZATION RESIDENTIAL SERVICES (SSRS) RFP FROM THE STATE OF HAWAII DEPARTMENT OF HEALTH BEHAVIORAL HEALTH ADMINISTRATION. KEY TOPICS THAT THE CAN IDENTIFIED AND ADDRESSED INCLUDED: I. IDENTIFYING AND ARRANGING A VENDOR FOR TRANSPORTATION SERVICES (STAN'S TAXI) FOR EXCEPTION FUNDING TRANSPORT NEEDS. II. IDENTIFYING FREE PROVIDERS OF CELLPHONES TO INCREASE ACCESS AND SERVICE TO THOSE WITHOUT PHONES OR COVERAGE. III. UNDERSTANDING SNAP SERVICES AND ELIGIBILITY BY INVITING SNAP REPRESENTATTIVES TO PRESENT AND SHARE INFORMATION DUE TO A LACK OF UNDERSTANDING OF SNAP OFFERINGS AND FOOD RESOURCES ACROSS EAST HAWAII. IV. UNDERSTANDING AND SHARING OF AGING AND DISABILITY SERVICES TO ASSIST THE ELDERLY AND OTHERS WHO QUALIFY FOR ADDITIONAL SERVICES.
