About this organization
Mission
To connect uninsured and underserved Stark County residents to health care resources.
About
Thrive: In 2016 Ohio had one of the worst infant mortality rates in the country, with Stark County having one of the worst rates in the state at 8.23. The racial disparity rate was 20.24 for nonhispanic blacks and 6.66 for nonhispanic whites. Thrive's goal was to use community resources, home visiting programs and education on safe sleep wo reduce those numbers. Thrive won a Nova Award in July 2019 for its work. Our current infant mortality rate is 6.0 with no racial disparity. We have 4 CHWs working as part of the Thrive initiative to reduce infant mortality. 7 CHWs focusing on helping clients with chronic diseases address their social determinants of health, get and bond with a primary care provider, and improve their health as indicated by an improvement in lab values began in 2019. The work of both Thrive and chronic disease CHWs is directed by the Pathways Care Coordination System and the Pathways Hub. The Hub serves as a data collection site for evaluation of the effectiveness of CHWs by Dr. Peter Leahey from Kent State and for reimbursement for completion of Pathways by the Medicaid Managed Care Organizations in Ohio. We continue to lead and host monthly peer support meetings for CHWs throughout Stark County.
