IN 2018 WE USED THE FUNDING FROM SHIN-NY/HCRA/MEDICAID TO SUPPORT STAFF AND TECHNOLOGY FOR THIS WORK. OUR SOLUTIONS & MEMBER ENGAGEMENT TEAM RECRUITS NEW PARTICIPANTS, ENSURES COMPLIANCE WITH STATE PRIVACY & SECURITY POLICIES AND PROVIDES ACCOUNT MANAGEMENT SERVICES. THE IT GROUP COVERED FOUR AREAS: INNOVATION, DEVELOPMENT, ENGINEERING AND SECURITY. [SEE CONTINUATION ON SCHEDULE O]THE INNOVATION TEAM INCLUDED EIGHT PROJECT MANAGERS, A BUSINESS ANALYST, A SOFTWARE QUALITY ASSURANCE ENGINEER A DIRECTOR OF EHR INTEROPERABILITY AND A VICE PRESIDENT OF INNOVATION. RECENT EVIDENCE SUGGESTS THAT HIV + PERSONS WITHOUT EVIDENCE OF TREATMENT FREQUENTLY VISIT HOSPITALS AND VARIOUS AMBULATORY SITES. RHIOS CAN FACILITATE CARE TRANSITIONS BY ALERTING PRIMARY CARE PHYSICIANS (PCP) AND CASE MANAGERS IN REAL TIME OF OPPORTUNITIES TO RETURN VULNERABLE HIV+ INDIVIDUALS TO CARE OF THEIR PCP. HEALTHIX WILL BUILD AN ENHANCED DETECTION, MONITORING, AND ALERTING CAPABILITY THAT WILL EXTEND ITS EXISTING INFRASTRUCTURE AND FUNCTIONALITY. THE PRINCIPLE OBJECTIVES OF THE PROJECT ARE:1. IDENTIFY PEOPLE WITH HIV/AIDS.2. DETERMINE WHICH PATIENTS WITH HIV/AIDS ARE LOST TO CARE.3. WHEN A PATIENT WHO IS LOST TO CARE VISITS A FACILITY, SEND A NOTIFICATION TO THAT FACILITY WHERE IT WILL BE ROUTED TO AN APPROPRIATE CARE GIVER. THE PROJECT MET ALL DELIVERABLES AND WAS COMPLETED IN SEPTEMBER OF 2018. THE COMPANY RECORDED $100,000 IN REVENUE IN CONNECTION WITH THIS GRANT.HGC #3 GATEWAY VENDOR BACKGROUND INCREASING DATA EXCHANGE WITH PHYSICIAN PRACTICES IS A CRITICAL INITIATIVE. HEALTHIX HAS A PROVEN TRACK RECORD IN REDUCING IMPLEMENTATION TIME FOR PARTICIPANTS BY DEVELOPING HUB MODEL INTEGRATIONS WITH OVER 16 EHRS THAT HAVE ADVANCED HL7V3 INTEGRATION CAPABILITIES, AND HEALTHIX CONTINUES TO IMPLEMENT HUB MODEL INTEGRATIONS WITH MORE EHRS. CONCURRENTLY, ONE OF THE KEY GOALS AND STRATEGIES FOR 2018 IS FOR HEALTHIX TO INCREASE CAPACITY AND SHORTEN INTEGRATION COMPLETION TIMES FOR NEW PARTICIPANTS THAT ARE USING EHRS THAT CANNOT BE CONNECTED TO HEALTHIX THROUGH A HUB MODEL INTEGRATION OR DO NOT HAVE A HUB THAT HEALTHIX CAN CONNECT TO. HEALTHIX HAS BEEN SUCCESSFUL IN OPTIMIZING OUR THROUGHPUT IN ONBOARDING NEW PARTICIPANTS AND WE CONSISTENTLY MAINTAIN A BACKLOG OF PROJECTS. THE ESTIMATED ANNUAL PROJECT VOLUMES ARE EXPECTED TO EXCEED OVER 100 PROJECTS. TO ADDRESS THIS ISSUE, HEALTHIX ENGAGED A GATEWAY VENDOR (GV) TO SUBSTANTIALLY INCREASE OUR CAPACITY TO CONNECT NEW PARTICIPANTS TO OUR HIE. WE IDENTIFIED A VENDOR TO DEVELOP SOLUTIONS TO PARTNER WITH HEALTHIX TO TAKE OVER THE RESPONSIBILITY FOR ACQUIRING CLINICAL DATA FROM EHR VENDORS THAT DO NOT QUALIFY FOR HUB MODEL INTEGRATIONS, STANDARDIZE THE DATA, AND PROVIDE HEALTHIX WITH A SINGLE C-CDA FEED USING IHE PROTOCOLS AND INCLUDING REGISTRATION AND CONSENT VIA ADT. THE HIGH-LEVEL REQUIREMENTS FOR THIS SERVICE ARE DESCRIBED BELOW: 1. ACCEPT DATA FROM EHRS IN MULTIPLE MODES OF TRANSPORT (DIRECT, SFTP, TLS, VPN, OTHER) AND MULTIPLE FORMS OF PAYLOAD (C-CDA, HL7V2, FLAT FILE). 2. VALIDATE DATA SOURCES. WE STRONGLY RECOMMEND THE USE OF SELF-SERVICE TOOLS FOR EHRS. 3. HOLD EHR VENDOR ACCOUNTABLE FOR SCOPE DEFINED BY HEALTHIX. 4. NORMALIZE DATA FROM ALL SOURCES AND TRANSFORM IT INTO A C-CDA THAT MEETS STRICT SYNTACTIC AND SEMANTIC STANDARDS DEFINED BY HEALTHIX. C-CDAS TRANSMISSION MUST FOLLOW IHE PROTOCOLS (E.G. XDS.B) AND USE ADT FOR RHIO REGISTRATION AND CONSENT. HEALTHIX WILL PROVIDE VALIDATION TOOLS OR DEVELOP WITH GV. 5. WORK WITH EHR, HEALTHIX TECHNICAL LEAD AND HEALTHIX PROJECT MANAGER TO COMPLETE INTEGRATIONS. 6. COMPLETE INTEGRATIONS WITHIN A FIXED PERIOD. PROJECT BEGINS WHEN THE EHR IS READY (QUALIFIED) AND ENDS WHEN THE PARTICIPANT HAS ACCEPTED THE CONSUMPTION OF THEIR DATA INTO HEALTHIX BY COMPLETING USER ACCEPTANCE TESTING IN PRODUCTION. 7. MAINTAIN ACTIVE DATA FEEDS WITH EHRS, MONITOR FOR OUTAGES AND PROACTIVELY REMEDIATE. 9. WORK WITH HEALTHIX PRODUCTION SUPPORT TO COORDINATE TROUBLESHOOTING ISSUES ALONG THE DATA FLOW FROM EHR TO GV TO HEALTHIX. IN THE HUB MODEL, THE EHR VENDOR MAINTAINS ONE INTERFACE TO HEALTHIX FROM ITS DATA CENTER OR INTERFACE ENGINE, WHICH SUPPORT MULTIPLE CLIENTS; AND THE VENDOR CAN EASILY ADD THE PARTICIPANT TO THE VENDOR'S EXISTING INTERFACE WITH HEALTHIX.THE GRANT PROJECT HAD THREE PHASES: (1) INCREASE RECRUITMENT AND IMPLEMENTATION OF PARTICIPANTS USING THE EXISTING 7 HUB MODEL EHRS, (2) CREATE HUB MODEL INTERFACES WITH AT LEAST 3 OF THE ADDITIONAL 5 EHRS, AND (3) RECRUIT NEW PARTICIPANTS WHO USE THE ADDITIONAL 5 EHRS. AT COMPLETION, THE GRANT RESULTS SURPASSED THE ORIGINAL GOALS OUTLINED IN THE PROPOSAL.RECRUITMENT: GOAL = 85, ACTUAL # = 108IMPLEMENTATION: GOAL = 105, ACTUAL # = 109NEW HUB INTERFACES: GOAL = 3, ACTUAL # = 5DESCRIPTION OF OUTCOMESPHASE 1: INCREASE RECRUITMENT AND IMPLEMENTATION OF NEW PARTICIPANTS WHO USE THE 7 CURRENT HUB MODEL EHRS.THE GOAL FOR THE FIRST PHASE OF THE ADDITIONAL FUNDING GRANT WAS TO RECRUIT 60 SITES, AND COMPLETE IMPLEMENTATIONS FOR 105 SITES. HEALTHIX WAS ABLE TO EXCEED THESE GOALS AND RECRUITED 73 NEW SITES (+22% OF GOAL), AND COMPLETED IMPLEMENTATIONS FOR 111 SITES (+6% OF GOAL).
