Provide preventive services and healthy lifestyle education to the community to combat heart disease, diabetes and hypertension. In 2018, services and education were provided to 6748 area residents through our New Beginnings education program, community access program, and our comprehensive chronic care free clinic for under-insured and uninsured. Community garden, farmer's market and farm gleanings netted over 17,600 pounds of fresh fruits and vegetables. Childhood obesity programming for families of 759 children. Senior Classes for 861 participants. Free clinic outcomes continue to exceed national averages.
