PATIENT, FAMILY, AND CAREGIVER SUPPORT - PROVIDES SUPPORT TO THE PERSON WITH DEMENTIA AND THEIR FAMILY MEMBERS OR CAREGIVERS AND PROVIDES TRAINING AND CASE CONSULTATION.THE OVERALL GOAL OF THE ADI-SSS FEDERAL GRANT PROJECT IS THE CREATION OF A SUSTAINABLE NETWORK, UTILIZING EXISTING RESOURCES WHEN POSSIBLE, WITHIN UNDERSERVED COMMUNITIES (6 NEW SERVICE COUNTIES) TO BETTER SUPPORT INDIVIDUALS WITH ALZHEIMER'S AND OTHER DEMENTIAS AND THEIR CAREGIVERS. THE OBJECTIVES ARE 1) TO PROMOTE THE HEALTH AND WELL-BEING OF PERSONS AFFECTED BY ALZHEIMER'S AND OTHER DEMENTIAS BY PROVIDING LINKS TO APPROPRIATE INFORMATION, MEDICAL DIAGNOSIS AND MANAGEMENT AND CAREGIVER SUPPORT SERVICES AND 2) TO LINK MEDICAL AND SOCIAL SERVICE SYSTEMS TO PROMOTE EARLY DIAGNOSIS AND TREATMENT AND INCREASE ACCESS TO APPROPRIATE MEDICAL AND SOCIAL SUPPORTS.ALZHEIMER FAMILY AND CAREGIVER SUPPORT PROGRAM (AFCSP) - THE GOAL IS TO ENABLE CLIENTS AND, WHEN APPROPRIATE, CLIENTS' FAMILIES TO GAIN ACCESS TO AFCSP AND RECEIVE A FULL RANGE OF APPROPRIATE SERVICES IN A PLANNED, COORDINATED, EFFICIENT, AND EFFECTIVE MANNER. SERVICES INCLUDE ASSESSMENT, CONSULTATION, CASE PLANNING, MONITORING AND REVIEW, ADVOCACY AND REFERRAL. A WAITING LIST IS MAINTAINED AND THESE FAMILIES ARE REFERRED TO OTHER HELPFUL SERVICES IN THE INTERIM. OVER 675 PEOPLE HAVE RECEIVED AFSCP SERVICES THROUGH THE ALLIANCE SINCE 2006.IN THE PAST YEAR OVER 6,104 PEOPLE HAVE BEEN HELPED BY: 1) PRESENTING 165 EDUCATION PROGRAMS ATTENDED BY 3,682 PEOPLE, FORM THE ABC'S OF ALZHEIMER'S AND DEMENTIA TO CROSSING BRIDGES, THE ALLIANCE'S EXCEPTIONAL PROGRAM FOR PEOPLE JUST DIAGNOSED WITH MEMORY IMPAIRMENT AND THEIR CAREGIVERS; 2) RESPONDING TO 985 HELPLINE CALLS AND 194 REQUESTS FOR IN-PERSON CARE CONSULTATIONS WITH FAMILIES; 3) PROVIDING 178 SUPPORT GROUP MEETINGS ATTENDED BY OVER 1,243 PEOPLE; AND BY 4) MAINTAINING SIX OUTREACH SPECIALISTS SERVING RURAL COMMUNITIES SO PEOPLE STRUGGLING WITH ALZHEIMER'S AND OTHER DEMENTIA DON'T ALWAYS HAVE TO TRAVEL TO MADISON OR MILWAUKEE TO RECEIVE HELP. THE OVERALL GOAL OF DEMENTIA CARE MANAGEMENT IS TO SUPPORT THE PERSON WITH DEMENTIA AND THEIR FAMILY MEMBERS OR CAREGIVERS AND PROVIDE TRAINING AND CASE CONSULTATION TO THE AGING SERVICE PROFESSIONALS. SERVICES INCLUDE EDUCATION, CONSULTATIONS, AND LINKAGE TO NEEDED RESOURCES. CARE CONSULTATIONS, PLANS FOR CARE AND FACILITY STAFF TRAINING ARE ALSO INCLUDED IN THIS CATEGORY. OVER 3,052 PEOPLE ARE SERVED IN DANE COUNTY.THE OVERALL GOAL OF THE PAT WILSON FUND RESPITE CARE GRANTS IS TO PROVIDE OPTIMAL CARE AND SERVICES TO THE INDIVIDUALS LIVING WITH DEMENTIA AND TO THEIR CAREGIVERS AND FAMILIES BY PROVIDING RESPITE DOLLARS DIRECTLY TO FAMILIES.
