Ongoing case management: The ongoing case management program provides case management services which focus on maintaining child safety while children are placed in out-of-home care and following reunification with birth parents along with achieving permanency for children in the most expeditious manner possible. Case managers continuously assess for safety at each home visit through active communication with all caregivers within the home. Along with assessing for safety, case managers conduct parental protective capacity assessment, develop service plans, refer families to formal and informal supports, monitor parental progress and prepare and participate in legal processes such as permanency hearings, extension hearings, and hearings regarding termination of parental rights. During 2018, 1169 families benefited from the program.
