ELIGIBLE SENIOR PARTICIPANTS ARE BARREN COUNTY RESIDENTS WHO ARE LOW INCOME SENIORS (200% POVERTY LEVEL OR BELOW), ASSETS UNDER 10,000 (DOESN'T INCLUDE HOME), WHO HAVE NO OTHER RESOURCES TO PAY FOR MEDICAL EXPENSES, AND ARE AGE 65 OR OLDER. SENIOR BENEFITS INCLUDE PAYMENT OF 75% OF PRESCRIPTION MEDICATIONS UP TO 200 PER MONTH. A SENIOR MUST RENEW ELIGIBILTY EVERY 6 MONTHS. COMMUNITY MEDICAL CARE PROVIDED ASSISTANCE TO 117 SENIORS. 1,283 VOUCHERS WERE WRITTEN FOR THE 117 SENIORS WHICH RESULTED IN 36,677 BEING SPENT BY COMMUNITY MEDICAL CARE FOR SENIOR MEDICATIONS. 8 OF THE 117 SENIORS COMPLETED A STARKEY HEARING FOUNDATION APPLICATION FOR THE HEAR NOW PROGRAM AND RECEIVED A HEARING TEST. COMMUNITY MEDICAL CARE PAID 80% OF THE APPLICATION FEE FOR THE 8 SENIORS COSTING 1,523. 3 OF THE 117 SENIORS RECEIVED A DENTAL EVALUATION AND WAS DETERMINED TO BE A GOOD CANDIDATE FOR THE DENTURE PROGRAM. COMMUNITY MEDICAL CARE PAID 3,250 FOR 3 UPPER AND 3 LOWER SETS OF DENTURES. ELIGIBLE ADULT PARTICIPANTS ARE BARREN COUNTY RESIDENTS WHO ARE LOW INCOME (200% POVERTY LEVEL OR BELOW), WORKING, ASSETS UNDER 5000 (NOT INCLUDING HOME),UNINSURED OR UNDERINSURED, AND AGE 18 TO 64. ADULTS RECEIVE PRIMARY HEALTH CARE (20 FIRST VISIT AND 10 ON FOLLOW-UP VISITS), PAYMENT OF 75% OF PRESCRIPTION MEDICATIONS UP TO 150 A QUARTER, DISCOUNT EYE EXAMS AND PRESCRIPTION GLASSES, DISCOUNT EMERGENCY ROOM DENTAL EXAM (X-RAY AND EXTRACTIONS ONLY), AND SPECIALIST CONSULTATION, COUNSELING, BASIC PHYISCAL THERAPY, HOSPITAL CARE- INPATIENT, OUTPATIENT AND CLINIC SERVICES. COMMUNITY MEDICAL DOES NOT PROVIDE THE MEDICAL SERVICES, THE LOCAL HOSPITAL, PHYSICIANS, DENTISTS, AND SPECIALIST VOLUNTEER THEIR SERVICES AT THEIR OFFICES. SOME SERVICES MAY NOT BE COVERED THROUGH COMMUNITY MEDICAL CARE AND THE PARTICIPANT WILL BE REPONSIBLE FOR SERVICES. AN ADULT MUST RENEW ELIGIBILTY EVERY 3 MONTHS. COMMUNITY MEDICAL CARE PROVIDED ASSISTANCE TO 7 ADULTS. 42 VOUCHERS WERE WRITTEN FOR THE 7 ADULTS WHICH RESULTED IN 575 BEING SPENT BY COMMUNITY MEDICAL CARE FOR ADULT MEDICATIONS. COMMUNITY MEDICAL CARE ASSISTED WITH 1 ADULT DENTAL APPOINTMENT, 1 ADULT EYE EXAM AND DISCOUNTED GLASSES.
