About

Erie Family Health Center (Erie) is a Federally Qualified Health Center, regional healthcare resource for close to 82,000 patients consisting of seven large primary care centers, the region's oldest and largest comprehensive Teen Center, and five school-based health centers. To empower our mission and vision and to fulfill Erie's values of dignity, impact, excellence, relationships, and learning, Erie provides a comprehensive array of health services at one convenient location to empower lifelong wellness among the community's most vulnerable members. Since growing from a one-room location in 1957, Erie's commitment to helping meet the needs of underserved residents has remained steadfast; more than 90% of patients are low-income, 71% are Hispanic and 27% are uninsured. Erie has been deliberate in its efforts to expand its locations into dynamic health centers that are uniquely positioned to meet the needs of community members. Erie health centers offer a wide range of high quality, affordable services, including integrated medical, oral, and behavioral healthcare; chronic disease management; vaccines and flu shots; HIV testing; STI testing/treatment; prenatal care; and women's health services, with programs intentionally designed to meet the needs of Erie's individual patients. Erie services provide culturally-sensitive, trauma-informed care for patients, with providers and front-line staff who are bilingual in Spanish and English, while offering translation services for those who prefer other languages. Erie will continue to invest in efforts to ensure staff are reflective of those it serves, which will, in turn, provide economic stimulus to the communities Erie serves. Erie continues to strive toward achieving the high-quality outcomes that have placed it in the top tier of health centers nationally, with the U.S. Health Resources and Services Administration recognizing Erie as a "Health Center Quality Leader" every year from 2014 to 2019. Further, Erie has been named a Top Chicago Workplace, an award given by the Chicago Tribune based on Erie employee feedback. According to the national Uniform Data System, which tracks performance of federally qualified health centers like Erie, Erie ranks in the top 10% of health centers nationally for our clinical performance, including rates of cervical and colorectal cancer screenings. Further, Erie delivers more babies than any other community health center in Illinois and ranks in the top 1% nationally for number of deliveries. At Erie, 94% of babies were born with normal birth weights, compared to 91% in Illinois and 92% nationally. Additionally, 83% of women at Erie receive prenatal care in the first trimester, compared to 75% in Illinois and 74% nationally.
