CVS Health Employee Relief Fund ('ERF') is organized to provide relief to employees of CVS Health Corporation and its affiliated entities ('CVS'). CVS has 250,000 eligible employees located in every state within the United States. The activities of ERF will be to provide short-term, immediate financial relief to CVS employees and family members who have suffered significant hardship as a result of natural disaster, family death, medical emergency or other unforeseen designated events and who have demonstrated a financial need. Employees can submit applications for funds needed to establish or re-establish a habitable and safe residence following a hurricane, fire, flood, tornado, etc., for funeral or travel expenses for attending a funeral or caring for a terminally ill employee or immediate family member, for initial assistance once an employee or spouse/domestic partner is deployed into the military, or for assistance with a personal or medical emergency or prescription requirements.
