LIVING WELL WITH A DISABILITY (LWWD) PROGRAM LWWD ASSISTED 135 CONSUMERS IN ACHIEVING AFFORDABLE, ACCESSIBLE HOUSING, ATTENDANT CARE AND COMMUNITY ACTIVITIES. NURSING HOME TRANSITION (NHT) SERVICES UNDER LWWD WORKED WITH 59 CONSUMERS IN NURSING HOME AND SUCCESSFUL TRANSITIONED 26 CONSUMERS FROM NURSING HOMES TO THE COMMUNITY. CILCP'S GRASSROOTS ADVOCACY GROUP, WHEELS IN MOTION (WIM) ADVOCATED TO THE CITY OF HARRISBURG FOR EQUAL ACCESS AND ADVOCATED AT THE STATE LEVEL FOR DESIGNATED CONSUMERS TO BE INVOLVED IN HCBS PROVIDERS "READINESS REVIEWS". CILCP WAS ALSO INVOLVED WITH THE PA DEPARTMENT OF HUMAN SERVICES, MANAGED CARE LONG TERM SERVICES & SUPPORTS COMMITTEE, CAPITAL AREA TRANSPORTATION ADVISORY, ADAPT, BRAIN INJURY ASSOCIATION, HOUSING AUTHORITY, LOCAL HOUSING OPTION TEAM, AND SECTION 811 ADVISORY COMMITTEE. DEAFBLIND LIVING WELL SERVICES (DBLWS) DBLWS MATCHED 48 CONSUMERS WITH SUPPORT SERVICE PROVIDERS (SSPS) FOR A TOTAL OF 989 ACTIVITIES TOTALING 3,617 HOURS OF SERVICE. ACTIVITIES INCLUDED SHOPPING, BANKING, MEDICAL APPOINTMENTS, COMMUNITY MEETINGS, FAIRS, AND FESTIVALS, AMUSEMENT PARKS, SPORTING EVENTS AND EXERCISING. DBLWS ALSO ADVOCATED FOR LEGISLATION AND CONTINUED FUNDING OF SSP SERVICES FOR PEOPLE WHO ARE DEAFBLIND. TRAINING VIDEOS (CAPTIONED) WERE PUBLISHED HIGHLIGHTING DEAFBLINDNESS, TECHNOLOGY, SSP TASKS, SERVICE DOGS AND SSP AND CONSUMER WHO IS DEABLIND IN ACTION AT THE GROCERY STORE. LIVING WELL TRANSPORT SERVICES (LWTS) CILCP PURCHASED A MV-1 COMMERCIAL ADA VAN TO PROVIDED ACCESSIBLE AND AFFORDABLE TRANSPORTATION AFTER 5:00 P.M. AND ON WEEKENDS, ON AN AS-NEEDED, FIRST COME, FIRST SERVED BASIS. CILCP BECAME LICENSED BY THE PUBLIC UTILITIES COMMISSION (PUC) IN SEPTEMBER 2017 TO OPERATE WITHIN DAUPHIN AND CUMBERLAND COUNTIES. HOME MODIFICATIONS & ACCESS SERVICES CILCP PROVIDED 138 CONSUMERS WITH HOME MODIFICATIONS SO THEY COULD LIVE WELL IN THEIR COMMUNITY. CILCP STAFF ALSO ADVOCATED FOR DAUPHIN COUNTY AFFORDABLE HOUSING ASSOCIATES AND DEVELOPERS TO RECEIVE FUNDING FOR 3 NEW HOUSING DEVELOPMENTS IN DAUPHIN COUNTY. SPECIALIZED SERVICES SPECIALIZED SERVICES IS A STATEWIDE PROJECT THAT PROVIDES TRAINING, ADVOCACY, COMMUNITY INTEGRATION AND OTHER TANGIBLE SUPPORTS TO ELIGIBLE NURSING HOME RESIDENTS ACROSS PENNSYLVANIA. 404 CONSUMERS RECEIVED SPECIALIZED SERVICES. SIX PEER COUNSELORS INTERVIEWED 2,400 PROGRAM PARTICIPANTS IN 2015/2017 FOR THE EXCEPTIONAL DURABLE MEDICAL EQUIPMENT GRANT PROGRAM. INDEPENDENT MONITORING FOR QUALITY (IM4Q) IM4Q IS AN INDEPENDENT PROGRAM CONTRACTED TO CONDUCT QUALITY OF LIFE SURVEYS TO INDIVIDUALS WITH INTELLECTUAL DISABILITIES. CILCP CONTRACTS WITH 3 ADMINISTRATIVE ENTITIES OR 6 COUNTIES (CUMBERLAND/PERRY, DAUPHIN, HUNTINGDON/MIFFLIN/JUNIATA). THE IM4Q PROGRAM USED INDEPENDENT CONTRACTORS TO AID IN SCHEDULING AND COMPLETING THE SURVEYS. 13 INDEPENDENT CONTRACTORS INTERVIEWED 290 INDIVIDUALS. AN INDIVIDUAL WHO WAS SURVEYED WISHED TO WORK MORE AND BUY A LAPTOP COMPUTER. HE BECAME AN INDEPENDENT CONTRACT, HAD A TEAM PARTNER AND WAS ABLE TO ACHIEVE HIS DESIRE. CONSULTATION & TRAININGS COLLEGES, UNIVERSITIES, CHURCHES AND HUMAN SERVICE AGENCIES BENEFITED FROM THE KNOWLEDGE OF CILCP'S ADA EXPERTS. NEARLY 200 HOURS OF ADA CONSULTATIONS AND ASSESSMENTS WERE PROVIDED. THE CILCP ALSO HOSTED TRAINING FOR THE ACCESSIBILITY SURVEY ACADEMY PROJECT (ASAP). SIXTEEN OF THE SEVENTEEN PA CENTERS FOR INDEPENDENT LIVING PARTICIPATED.
