GENERAL PROGRAM SERVICE INFORMATION GEISINGER COMMUNITY HEALTH SERVICES (GCHS) WAS ESTABLISHED IN JULY 1998 AS A 501(C)(3) NOT-FOR-PROFIT CORPORATION TO PROVIDE COMPREHENSIVE COMMUNITY BASED HEALTH SERVICES THROUGHOUT NORTHEASTERN AND CENTRAL PENNSYLVANIA. THE ORGANIZATION EMPLOYS APPROXIMATELY 297 INDIVIDUALS THAT PROVIDE AN ARRAY OF SPECIALIZED MEDICAL SERVICE PROGRAMS. GCHS IS LOCATED IN DANVILLE PA, WITH BRANCH FACILITIES THROUGHOUT CENTRAL AND NORTHEAST PENNSYLVANIA AND THE POCONOS. GCHS PROVIDES QUALITY MEDICAL HEALTHCARE REGARDLESS OF RACE, CREED, SEX, NATIONAL ORIGIN, HANDICAP, AGE OR ABILITY TO PAY. PROGRAM SERVICE ACTIVITIES THAT ARE AVAILABLE THROUGH GCHS INCLUDE THE FOLLOWING: GEISINGER HOME HEALTH & HOSPICE IS A FULL-SERVICE HOME HEALTH AGENCY WITH OFFICES IN DANVILLE, WILKES- BARRE, NORTHUMBERLAND, LEWISBURG, MONTOURSVILLE, LEWISTOWN AND CAMP HILL THAT PROVIDES HOME HEALTH SERVICES TO PATIENTS SKILLED NURSING, HOME HEALTH AIDE, MEDICAL SOCIAL WORK, AS WELL AS PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY SERVICES WHICH ARE AVAILABLE 24 HOURS/7 DAYS A WEEK. THE AGENCY IS BOTH MEDICARE AND MEDICAID CERTIFIED, AND THEREFORE MAY PROVIDE HOME CARE SERVICE TO MOST PAYOR CATEGORIES. IN ADDITION, THE AGENCY PROVIDES MATERNAL/CHILD HEALTH SERVICES IN MOST AREAS. THE HOSPICE PROGRAM PROVIDES CARE TO THE PATIENT WHO IS TERMINALLY ILL WITH A LIFE EXPECTANCY OF 6 MONTHS OR LESS. BASE SERVICES INCLUDE: SKILLED NURSING, HOSPICE AIDE, MEDICAL SOCIAL WORK, SPIRITUAL CARE, AND BEREAVEMENT. IN ADDITION, PHYSICAL THERAPY, OCCUPATIONAL THERAPY, AND SPEECH THERAPY ARE UTILIZED WHEN THE PATIENT CAN BENEFIT FROM THOSE DISCIPLINES. NEEDS OF THE HOSPICE PATIENT FAMILY ARE ALSO ADDRESSED BY THE HOSPICE TEAM, IE. CARE GIVER STRESS, DEPRESSION, ANXIETY. HOSPICE SERVICES ARE PROVIDED TO PATIENTS IN CENTRAL PENNSYLVANIA AND ARE AVAILABLE 24 HOURS A DAY. PATIENTS ARE SERVED IN THEIR PLACE OF RESIDENCE - THEIR HOME, SKILLED NURSING FACILITY OR ASSISTED LIVING FACILITY. THE HOSPICE PROGRAM IS BOTH MEDICARE AND MEDICAID CERTIFIED, AND THEREFORE MAY PROVIDE HOSPICE SERVICES TO ALL PAYOR CATEGORIES. VITALINE HOME INFUSION THERAPY SERVICES: LICENSED AS A PHARMACY PROVIDER, VITALINE PROVIDES TOTAL PARENTERAL AND ENTERAL NUTRITION THERAPIES, ANTIBIOTIC, CHEMOTHERAPY AND A VARIETY OF OTHER INFUSION RELATED THERAPIES TO PATIENTS IN CENTRAL AND EASTERN PENNSYLVANIA WHO MAY RESIDE AT HOME OR IN SKILLED NURSING FACILITIES. SERVICES ARE PROVIDED FOR ALL PAYOR CATEGORIES. THIS PROGRAM WILL BE TRANSITIONED TO GEISINGER CLINIC EFFECTIVE JULY 1, 2018. LIFE GEISINGER: BASED ON A FEDERAL AND STATE SPONSORED HEALTH CARE MODEL, PROGRAMS FOR ALL-INCLUSIVE CARE OF THE ELDERLY, NATIONALLY KNOWN AS PACE. THE MAIN PREMISE IS TO SERVE FRAIL ELDERLY WITHIN THE COMMUNITY WHO MIGHT OTHERWISE BE CONFINED TO A NURSING HOME. IT PROVIDES COMPREHENSIVE HEALTH CARE SERVICES TO MEET THE PARTICIPANTS' MEDICAL, SOCIAL, AND EMOTIONAL NEEDS. THE PROGRAM HAS TWO FULL LIFE CENTERS LOCATED IN SCRANTON AND KULPMONT AS WELL AS AN ALTERNATE CARE SITE IN WILKES-BARRE. THE GEISINGER CENTRAL PA HEALTH CARE QUALITY UNIT (HCQU) WORKS IN CONJUNCTION WITH INTELLECTUAL/DEVELOPMENTAL DISABILITY PROGRAMS IN 13 CENTRAL PENNSYLVANIA COUNTIES TO PROVIDE HEALTH RELATED TRAINING AND TECHNICAL ASSISTANCE TO CAREGIVERS OF AND PERSONS WITH INTELLECTUAL/DEVELOPMENTAL DISABILITIES LIVING IN PENNSYLVANIA. OUR PURPOSE IS TO BUILD CAPACITY AND COMPETENCY IN THOSE RECEIVING CARE AND THOSE PROVIDING CARE. THE GOAL OF THE HCQU IS TO ASSURE THAT INDIVIDUALS WITH INTELLECTUAL/DEVELOPMENTAL DISABILITIES LIVING IN PENNSYLVANIA ARE PROVIDED WITH THE FINEST COMMUNITY SERVICES, HAVE ACCESS TO QUALITY HEALTH CARE ASSURING THE BEST POSSIBLE PHYSICAL AND BEHAVIORAL HEALTH SO THEY CAN PARTICIPATE FULLY IN COMMUNITY LIFE. THE HCQU ALSO WORKS IN CONJUNCTION WITH THE BEHAVIORAL HEALTH PROGRAMS OF NORTHUMBERLAND COUNTY AND CMSU JOINDER TO PROVIDE WELLNESS NURSE SERVICES THAT PROVIDE PHYSICAL HEALTH AND WELLNESS COACHING TO PERSONS WITH SERIOUS MENTAL ILLNESS AND CO-MORBIDITIES. THE HCQU ALSO PROVIDES OVERSIGHT TO A CONTRACT THAT PROVIDES SERVICES BY AN RN AND MEDICAL ASSISTANT TO THE NORTHUMBERLAND COUNTY COMMUNITY OUTREACH MEDICATION CLINIC FOR PERSON WITH INTELLECTUAL DISABILITIES AND BEHAVIORAL HEALTH DISORDERS II. UNCOMPENSATED CARE GCHS RECOGNIZES THAT ITS MISSION IS TO MEET COMMUNITY HEALTH NEEDS AND TO SUPPORT THE GOALS OF GEISINGER IN THE COMMUNITIES WHERE GCHS PRACTICES. IN THIS REGARD, GCHS PROVIDES FREE OR SUBSIDIZED CARE BELOW COST AND SUPPORTS VARIOUS HEALTH ACTIVITIES AND PROGRAMS IN SUPPORT OF THE COMMUNITY. A. CHARITY CARE THE UNREIMBURSED COST OF CHARITY CARE REPRESENTS THE COST GCHS INCURS BY PROVIDING FREE OR DISCOUNTED SERVICES TO THOSE WHO CANNOT AFFORD TO PAY. FOR THE FISCAL YEAR ENDED JUNE 30, 2018, THE COST OF THE CHARITY CARE WAS 59,931. B. MEDICARE/MEDICAID IN RECOGNIZING ITS MISSION TO THE COMMUNITY, GCHS PROVIDES CARE, BELOW COST, TO PERSONS COVERED BY GOVERNMENTAL PROGRAMS. TO THE EXTENT REIMBURSEMENT IS BELOW THE COST OF PROVIDING HEALTHCARE, GCHS IS FURTHERING ITS MISSION TO THE COMMUNITY. THE UNREIMBURSED VALUE OF MEDICARE AND/OR MEDICAID IS EQUAL TO THE COST OF PROVIDING SERVICES LESS THE AMOUNT RECEIVED AS REIMBURSEMENT UNDER THE PROGRAM. THE UNREIMBURSED VALUE OF PROVIDING CARE TO THESE PATIENTS WAS 4,152,215 DURING THE FISCAL YEAR ENDED JUNE 30, 2018. C. OTHER UNCOMPENSATED PATIENT SERVICES IN ADDITION, GCHS PROVIDES SERVICES FOR WHICH FULL PAYMENT IS NOT RECEIVED. THE UNCOMPENSATED COST OF PROVIDING SUCH PATIENT SERVICES DURING THE FISCAL YEAR ENDED JUNE 30, 2018, WAS 254,249. III. COMMUNITY HEALTH, EDUCATION, AND OUTREACH GCHS SERVES THE COMMUNITY THROUGH GIFT-IN-KIND CONTRIBUTIONS. THE PUBLIC AND VARIOUS CHARITABLE ORGANIZATIONS LOCATED THROUGHOUT NORTHEASTERN AND CENTRAL PENNSYLVANIA RECEIVED APPROXIMATELY 14,931 GIFTS-IN-KIND DURING THE FISCAL YEAR ENDED JUNE 30, 2018. IV.
