This fresh start program offered 105 transitional housing beds to veterans as well as case management and other supportive services to 316 veterans in FY18. The program well exceeded the mandated VA outcome target of 65% of veterans discharged moved onto permanent housing placements with 80% of all veterans in the fresh start program moving to permanent housing. Only 3.3% of all veterans discharged were for disciplinary reasons or left the program against advice (VA target was 23%), and 16.7% of the veterans discharged were placed in higher levels of care. 85% of all veterans who were able to work were employed at discharge (VA target was 50%). Based on these FY18 accomplishments and our FY18 VA GPD grant applications the program was awarded additional funding through FY19.
