Our SNF offers short term rehabilitation, long term care, sub-acute services, respite care, palliative and in-patient hospice to the older adults of our community. Our care is person-centered, focusing on all the needs of each individual - physical, spiritual, psychological and social. We had 454 admissions in 2018. We discharged 442 residents back to their homes. We rendered 80,492 days of resident care, continued to extend the boundaries of the home to the community at large, sharing expertise, programs and resources through internship programs, community information programs, health fair participation and other outreach efforts.
